摘  要:目的探讨二维超声产前诊断胎儿腭裂的声像图特征,提高胎儿腭裂的产前检出率。方法采用二维超声对26800例孕14~42周妇女常规进行胎儿口鼻结构的探测,对从中检出的50例唇裂胎儿上颌骨牙槽嵴行横切面、矢状切面,硬腭行经鼻腔软骨部和经口裂的斜冠状切面、矢状切面,经颌下的冠状切面、矢状切面探测。结果本组病例经随访自然分娩及引产出唇腭裂畸形儿共56例,其中22例单纯唇裂和28例唇裂合并腭裂均检出,4例不完全性腭裂检出1例,2例软腭裂均未检出,1例单纯唇裂诊断为合并不完全性腭裂。二维超声产前诊断胎儿腭裂的检出率为88.2%,符合率89.3%,特异性95.5%,敏感性85.3%。结论二维超声显像对胎儿牙槽嵴裂、完全性腭裂的检出率和诊断准确性均较高,对不完全性腭裂和软腭裂难以检出。Objective To discuss the features of fetal cleft palate (CP) on two-dimensional uhrasonography (2DUS), in order to improve the accurate rate of fetal CP. Methods We examined the fetuses midface structures with 2DUS in 26800 women who were in 14 to 42 gestational weeks o There were 50 fetal cleft lips (CL)were checked out. The sagittal planes through transmaxillary palatine process, oblique transnasal gristle coronal axial planes, oblique transoral rimal coronal axial planes and transsubmental planes were scanned for CP. Results There were 56 fetuses with CL and CP who were verified by mature or induced labour. Twenty-two cases were with single CL and 28 CL and CP fetuses were diagnosed, but 1 of 4 incomplete CP and 2 of the single CP were missed, one case of single CP was diagnosed as combined incomplete CP prenatally. The detection rate, accuracy, specificity and sensitivity of CP on 2DUS were 88.2% (30/34), 89.3% (50/56), 95.5% (21/22), 85. 3%(29/34), respectively. Conclusions Most of fetal CP can be checked out ultrasonographically. Alvelolar crest cleft, complete middle CP can be clearly displayed and accurately diagnosed. As to incomplete and single CP, the diagnostic accuracy should be improved.
