急性冠状动脉综合征患者血栓前体蛋白和心肌肌钙蛋白I联合检测的意义

摘　　要：目的:了解血浆血栓前体蛋白和心肌肌钙蛋白I联合检测对急性冠状动脉综合征的临床价值。方法:利用酶联免疫分析法测定58例急性冠状动脉综合征患者,根据临床症状分为不稳定型心绞痛组(UAP组)和急性心肌梗死组(AM I组),并测定胸痛发作6 h内和24 h血浆血栓前体蛋白含量,免疫化学发光法测定其心肌肌钙蛋白I含量,同时检测相关指标CK-M B含量。结果:胸痛发作6 h内血栓前体蛋白AM I组为(11.73±4.38)μg/mL,UAP组为(3.20±2.10)μg/mL,AM I组明显高于UAP组,两组比较差异有统计学意义(P<0.01);心肌肌钙蛋白I:AM I组为(32.7±5.74)μg/L,UAP组为(24.8±2.72)μg/L,AM I组高于UAP组(P<0.05)。24 h时两组心肌肌钙蛋白I含量均持续升高,而血栓前体蛋白已基本趋于正常。结论:血栓前体蛋白可提示急性心肌梗死患者血栓形成于早期阶段,而心肌肌钙蛋白I对急性心肌梗死的诊断具有显著的特异性。两者联合检测可提高早期鉴别诊断的价值。Objective To explore the value of acute coronary syndrome with combining detection of thrombus precursor protein and cardiac troponin Ⅰ. Methods Fifty-eight patients with acute coronary syndrome were divided into two groups, unstable angina pectoris group （UAP group） and acute myocardial infarction group （AMI group）, to analyze thrombus precursor protein and cardiac troponin Ⅰ with enzyme-linked immunoassay in 6 hours, 24 hours after chest pain. And CK-MB contents were detected at the same time. Results In 6 hours after chest pain, the thrombus precursor protein contents was （11.73±4.38）μg/mL in AMI group, and（3.20±2. 10） μg/mL in UAP group. The cardiac troponin Ⅰ contents was （32.7±5.74）μg/mL in AMI group, and （24.8±2.72）μg/mL in UAP group. The numerical value of AMI group was more than UAP group （P〈0.05）. In 24 hours, the content of cardiac troponin Ⅰ kept increasing while thrombus precursor protein had no alteration in UAP group. Conclusion The content of thrombus precursor protein is the cues of thrombogenesis in the beginning, while cardiac troponin Ⅰ has distinguished specificity for the diagnosis of acute myocardial infarction. To detect these two indexes can elevate the value of antidiastole in the earlier period.
