岩舒联合斯奇康治疗恶性胸腹水疗效观察

摘　　要：目的:观察岩舒联合斯奇康治疗恶性胸腹水疗效。方法:将44例恶性胸腹水患者随机分为两组,试验组24例抽取胸腹水后腔内灌注岩舒和斯奇康:岩舒胸腔每次灌注20ml,腹腔每次灌注40ml;斯奇康胸腹腔灌注均为每次18ml,每周1次,连续2～4周。对照组20例采用单药顺铂60mg胸腔内灌注,100mg腹腔内灌注。结果:试验组16例恶性胸水中,有效率(CR+PR)87．5%,8例恶性腹水中,有效率75．0%,总有效率83．3%(20/24);试验组24例中,生活质量提高14例,稳定8例,降低2例。对照组15例恶性胸水中,有效率53．3%,5例恶性腹水中,有效率40．0%,总有效率50．0%(10/20);对照组20例中,生活质量提高5例,稳定6例,降低9例。两组比较P＜0．05,试验组优于对照组,无明显不良反应。结论:岩舒联合斯奇康治疗恶性胸腹水疗效可靠,生活质量提高,无明显不良反应,尤其适合治疗晚期不能耐受腔内化疗的患者。Objective：To observe the curative effect of ganshu combined with polysaccharide and nucleic acid （BCG-PSN） in the treatment of malignant succus in the thorax and belly cavities.Methods：44 patients with malignant succus in the thorax and belly cavities were randomly divided into experiment group （n=24） and control group （n=24）.The experiment group was given yanshu injection 20 ml combined with polysaccharide and nucleic acid 18 ml once a week by intrapleural perfusion for 2 to 4 weeks and ganshu injection 40 ml combined with polysaccharide and nucleic acid 18 ml onnce a week by intraperitoneal perfusion for 2 to 4 wecks;the control group was given cisplation alone bomg or 100 mg once a week by intrapleural perfusion or intraperitoneal perfusion respectively for 2 to 4 weeks. Results：In the experiment group,the effective rate was 87.5% in 16 patients with malignant hydrothorax and 75.0% in 8 patients with malignant aseites,the total effective rate was 83.3% （20/24）.The quality of life was improved in 14 cases,stable in 8 cases and lower in 2 cases.In the control group,the effective rate was 53.3% in 15 patients with malignant hydrothorax and 40.0% in 5 patients with malignant ascites,the total effective rate was 50.0% （10/20）.The quality of life was improred in 5 cases,stable in 6 cases and lower in 9 cases.The effective rate in the experiment group was significantly higher than that in the control group （P〈0.05）,and serious toxicity and side effects were not found.Conclusion：The curative effect of ganshu combined with polysaccharide and nucleic acid in malignant succus in thorax and belly cavities is rellable.It can improve patients＇quality of life without obvious noxious and side effects.especiaUy suitable for the late patients who could not tolerate the intracavitary chemotherapy.
