药物洗脱支架对冠心病患者介入术后炎症因子的影响

摘　　要：目的：比较药物洗脱支架与普通支架植入前后冠心病患者血超敏c-反应蛋白（hs-CRP）、白细胞介素-6（IL-6）、P选择素（P-selectin）、可溶性细胞间粘附分子（sICAM-1）的变化，探讨其意义。方法：选择有支架植入指征的冠心病患者66例，随机分为药物支架组和普通支架组，30例置入Cypher（Cypher支架组），36例置入普通Bxsonic支架（普通支架组）。分别于手术前，术后即刻，术后6h、术后24h、7d取肘静脉血，采用ELISA法检测IL-6、P选择素、s1CAM-1、hs-CRP。结果：Cypher支架组与普通支架组比较P选择素、IL-6、sICAM-1、hs-CRP水平在扩张前、术后即刻没有显著性差异（P〉0．05）而术后6h、24h、7d时Cypher支架与普通支架比较测定值低且差异有显著性（P〈0．05。0．01）。结论：Cypher药物洗脱支架对冠心病患者介入术后血炎症因子的产生有抑制作用，可能抑制介入术后的炎症反应是其抗再狭窄的机制之一。Objective： To compare the effects of drug eluting stent versus common stent on the levels of serum high sensitive C -reactive protein（ hs -CRP）, interleukin -6（IL -6）, plasma P- selectin and soluble intercellular adhesion molecule - I （ sICAM - 1 ） in patients with coronary heart disease before and after angioplasty. Method： 66 patients had been done coronary angiograph and divided randomizely into drug -e- luting stents implantment group （30 eases of Cypher stent implantment ） and common stents implantment group （ 36 eases of common stent implantment ）. Peripheral venous blood samples were taken before, immedi- ately,at 6,24 hours, seven days after angioplasty. The levels of hs - CRP , sICAM - 1, IL -6 and P - se- leetin were analyzed by enzyme linked immunosorbent assay（ELISA）. Result： There were no significant difference between the two groups at before and immediatety on levels of hs - CRP , sICAM - 1, IL - 6 and P - seleetin. At 6 hours, 24 hours and seven days after angioplasty, the levels of hs - CRP , slCAM - 1, IL -6 and P - seleetin were significantly lower in Cypher group than those in common stent group. Conclusion： The Cypher drug - eluting stents can reduce the levels of inflammatory factors after angioplasty. The results suggested that suppressing inflammatory factors maybe one of reasons for lowing restenosis.
