庆大霉素和利福平高渗盐水对烧伤肉芽创面治疗效果的比较

摘　　要：目的比较庆大霉素和利福平高渗盐水对烧伤肉芽创面的治疗效果。方法将112例烧伤肉芽感染创面患者,随机分为A,B2组。A组43例,采用庆大霉素高渗盐水纱布处理创面;B组69例采用利福平高渗盐水纱布处理创面。比较2组治疗前后病原菌检出率及烧伤肉芽创面植皮后成活率。结果治疗后A组病原菌检出率明显高于B组(P<0.05)。A组植薄中厚皮,全部成活29例,皮片成活面积在96%以上14例,散在1cm×1cm残余创面≤3个;B组植薄中厚皮,全部成活63例,皮片成活面积在96%以上6例,散在1cm×1cm残余创面≤3个。2组比较差异有显著意义(P<0.05)。结论利福平高渗盐水纱布湿敷创面优于庆大霉素高渗盐水纱布处理创面,具有高效、省时的特点,易于在临床上推广。Objective To compare the curative effect of two different processing methods to the infection granulation wound surface before operation, and seek one better method processing the infection granulation wound surface, Methods 112 patients were randomly divided into two groups, The infection granulation wound surface of 43 patients was treated by Gentamicin and 3% hypertonic saline absorbent gauze（Gentamicin group） ,and the others were treated by Rifampicin and 3% hypertonic saline absorbent gauze（rifampicin group）, The dressings were changed 1 ~ 2 times every day .All the patients were treated by skin gratting. 62 examples were flame burn patients,25 examples were burned by the aluminum, the iron it he water scald, 34 examples wore the hydrothermat scalds. Results In the Gentamicin group, 10- 14 days later, the infection granulation wound surface of 29 patients became clean, no exudation, no edema and ruddy; a small amount of secretion existed at wound surface in 14 cases. After skin grafting, the grafting skin survived completely in 29 patients, there were sporadic wound surface in all patients. In the rifampicin group,7-9 days later, the infection granulation wound surface of 63 patients became clean, no exudation, no edema; a small amount of secretion existed at wound surface in 3 Cases.After skin grafting,the grafting skin survived completely in 63 patients, there were sporadic wound surface in 3 patients.There were significant difference btween two groups （ P 〈 0.05）. Conclusion Two methods all are effective, but the effects of rifampicin group are more obvious than those of Gentamicin group.
