肺功能测定在儿童阻塞性睡眠呼吸暂停低通气综合征诊断中的应用

摘　　要：目的观察阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患儿的潮气呼吸、通气功能、脉冲震荡(IOS)指标的改变并探讨其在诊断OSAHS中的应用价值。方法将164例鼾症患儿按有无呼吸暂停分为两组,采用MS-IOS肺功能仪测定,先进行通气功能、IOS测定,再于睡眠时进行IOS、潮气呼吸分析测定。主要测定VPEF/VE、tPTEF/tE、VT、TI/tE、TI/Ttot、VT/TI;Z5、R5、Fres、R35、R5-R20、X5;FEF50/FIF50。结果两组TI/tE、TI/Ttot、FEF50/FIF50、R35较正常对照组增高,组VT、VT/TI较正常对照组减低(P<0.01)。与组比较,组VPEF/VE、tPTEF/tE、TI/tE、TI/Ttot增高,VT、VT/TI减低(P<0.01),并有呼吸暂停、SpO2下降。结论VPEF/VE、tPTEF/tE、TI/tE、TI/Ttot、FEF50/FIF50、R35是上呼吸道阻塞的指标,VT/TI可反映病变的严重程度。肺功能测定是诊断OSAHS的有效筛选方法。Objective：To evaluate the effect of pulmonary function test on the snoring children. Methods ：One hundred sixty-four patients were divided into two groups based on the physical examination, Pulmonary function was measured in 164 patients. Age-matched healthy infants were enrolled as controls. The parameters included VPEF/VE, tPTEF/tE, VT, TI/tE, TI/Ttot, VT/TI ; Z5, R5, Fres, R35, X35, R5-R20, X5; FEF50/FIF50. Results： TI/tE, TI/Ttot, FEF50/FIF50, R35 were significantly higher in two groups compared with the controls （P 〈 0. 01）. VT/TI. VT were significantly decreased in suspected groups compared with the controls （P〈0. 01）. Conclusion：The changes of breathing mechanics and severity degree can be reflected by pulmonary function test. VPEF/VE, tPTEF/tE,TI/tE,TI/Ttot, FEF50/FIF50, R35may be used as markers for upper airway obstruction. Pulmonary function test can play an important role in screen diagnosis of OSAHS.
