艾本合并化疗治疗骨转移癌疼痛的临床疗效

摘　　要：目的观察艾本（伊班膦酸钠）联合化疗治疗骨转移癌疼痛的临床疗效。方法70例骨转移癌的患者，随机分为对照组即单纯化疗组34例和化疗艾本综合治疗组36例，两组病例的临床资料具有可比性。以对照组化疗方案为参考，治疗组在化疗结束后3～5天内给予艾本3～4mg，溶于生理盐水500ml中，缓慢静脉滴注（至少4h），每4周重复1次，连续用4次后评价疗效。按WHO规定评价疗效及不良反应。结果局部疼痛控制情况：治疗组总有效率为77．8％，对照组为29．4％，两组比较有统计学差异（P〈0．05）；治疗后活动功能改善情况：治疗组总有效率为71．4％，对照组为16．7％，两组比较有统计学差异（P〈0：05）；骨转移灶控制情况：治疗组总有效率为63．9％，对照组为20．6％，两组比较有统计学差异（P〈0．05）；不良反应为一过性全骨疼痛及类感冒症状。结论艾本联合化疗治疗骨转移癌疼痛，止痛效果确切，而且骨转移灶明显缓解，提高活动能力，不良反应小，值得临床推广。Objective To observe curative effect of metastatic bone pain treated by chemotherapy combined with ibandronate. Methods 70 cases with metastatic bone pain were randomly divided into simple chemotherapy group （34 cases） and chemotherapy group combined with ibandronate （36 cases）. Clinical data of two groups had comparability. Evaluation of curative effect： control group took standard chemotherapy project but combined treatment group took chemotherapy project plus ibandronate. After finishing chemotherapy3 - 5d 36 cases of combined treatment group were treated with ibandronate 3 - 4mg in NS 500ml by intravenous infusion once 4 weeks for 4 months. The effect was evaluated when four cycles finished. The standard of WHO was used to compare the efficacy and the toxicity of chemotherapy and chemotherapy plus ibandronate. Results Local pain control： in treatment group total effective rate 77. 8% ,that of controls 29. 4% ,thereg statistics difference（ P 〈0. 05 ） ;after treatment, activity of daily living （ADL） in treatnent group is 71.4%, in controls is 76. 7%, there g statistic difference（ P 〈0. 05）. Bone metastasis：total affective rate of treatment group is 63.9% ,in controis is 20. 6% ,thereg statistic difforence（ P 〈0. 05 ）. There was a great difference between two groups by statistical analysis. The major adverse effects include fever, headache, fatigue, nausea and skeletal pain. All of the adverse effects are mild, and no intervention is involved. Conclusion Chemotherapy Combined with ibandronate is effective for relieving pain, repairing bone metastasis and improving ADL. The side - effect was mild to tolerate.
