自体血液回收对凝血功能影响的研究

摘　　要：目的对比自体血液回收机处理前后静脉血血细胞数量和凝血功能的变化。方法68例急性胸腹闭合性损伤合并内出血患者,分别检测自体血液回收前后血红蛋白(Hb)、红细胞数(RBC)、红细胞压积(Hct)、血小板(P lt)、凝血酶原时间(PT)、部分凝血酶原时间(APTT)、纤维蛋白原(FIB)、凝血酶时间(TT)。结果①当血液丢失于胸腹腔未经自体血液回收机处理前,患者Hb、RBC、Hct、P lt、FIB明显降低(P<0.01),PT、APTT、TT明显延长(P<0.01),其中以血小板降低最为明显。②经自体血液回输后,RBC及Hct明显升高(P<0.01),TT、APTT进一步延长(P<0.05),P lt、FIB进一步降低(P<0.05)。结论自体血液回收机处理前的静脉血中参与凝血的血小板和凝血因子大部分被消耗,经血液回收机处理回输给机体的仅等同于成分输注红细胞压积,患者术后不同程度的出现凝血功能改变。Objective To contrast the vein blood cell quantity and the blood coagulation function before and after the machine processing. Methods 68 chest and bellies internal bleeding patients, examine respectively the hemoglobin （ Hb ）, red blood cell （ RBC ）, Hct, blood platelets （Ph）, PT, the partial coagulation enzyme original time （AFVF）, fibroprotein（ FIB）, the blood coagulation enzyme time （tiT）. Results ①Before the prosessing of body blood recovery machine, suffer Hb, RBC, Hct, Plt, FIB lower obviously （ P 〈 0. 01 ） , obvious extension of PT, APTT, TT （ P 〈 0. 01 ） among them, the blood platelets lower the most obvious. ②After the body blood returned, the RBC and Hct go up obviously （ P 〈 0. 01 ） , further extension of tiT, AFVF （ P 〈 0. 05 ）, the Ph, FIB further lower （ P 〈 0. 05 ）. Conclusion Most part of the blood platelets and blood coagulation were consumed, blood recovered back to body only equaled to the Hct, the patient after operation have different degree of blood coagulation function change in the vein blood.
