腹腔镜下输卵管妊娠手术的临床疗效评价

摘　　要：目的：探讨各种类型输卵管妊娠的腹腔镜手术方法及临床效果。方法：应用不同的腹腔镜手术方式治疗128例输卵管妊娠患者，其中输卵管部分切除55例，输卵管开窗术38例，输卵管挤压取胚术8例，输卵管病灶切除术24例，子宫角切除术3例。结果：128例输卵管妊娠患者均通过腹腔镜完成手术，同时进行输卵管系膜囊肿剥除术15例，盆腔粘连松解术50例，输卵管造口术14例，卵巢囊肿剔除术2例，输卵管结扎术4例。手术时间30～90min，平均54min。术后盆腔留置引流管，12h拔除，术后无须留置尿管，术后12～24h肛门排气。血p-HCG24h下降≥50％，无持续性异位妊娠发生。结论：各种类型输卵管妊娠均可以通过腹腔镜完成手术，而且微创、安全。Objective：To study the operative methods and clinical effect of various laparoscopic tubal pregnancy operations. Mcthods：128 cases with tubal pregnancy were treated by laparoscope,in 55 cases of which partion uterine tubes were resected, in 38 cases uterine tubes were fenestrated in,8 cases uterine tubes were crushed and were embryo fetch,in 24 cases uterine tube lesions and in 3 cases comu uteri were resected. Results： 128 cases with tubal pregnancy were treated successttdly by laparoscope, at the same time, 15 cases underwent mesosalpinx cystomy,50 cases underwent lysis of pelvic adhesions, 14 cases underwent salpingostomy,2 cases underwent ovarian cyst enuclearion,4 cases underwent ligation of uterine tube. Operative time was 30 to 90 minutes, mean 54 minutes. Drainage tube was placed in pelvic carity,which should be pulled out after 12 hours. The time of passage of gas by anus was 12 to 24 hours after operation, β-HCG in the blood decreased more than fifty percent , no persistent ectopic pregnancy occured. Conclusions：Va- rious kinds of tubal preganacy can be treatecl with laparoscope and it is proved minimally invisive and safe.
