精子的获取方式和数量对卵浆内单精子注射治疗男性不育疗效的影响

摘　　要：目的:探讨获取不同的精子来源和数量与卵浆内单精子注射(ICSI)临床结局的关系。方法:比较在IC-SI周期使用不同精子来源(射出精子组比附睾和睾丸精子组)和精子数量(严重少精组比极度少精组)其受精率、卵裂率、优质胚胎率及临床妊娠率。结果:总受精率70.8%,卵裂率95.7%,优质胚胎率47.1%,31个周期获得临床妊娠(35.6%)。射出精子和附睾、睾丸精子两组的受精率、卵裂率、优质胚胎率、临床妊娠率均无显著性差异(P>0.05)。极度少精组的ICSI操作时间比严重少精组要长(P<0.05),但两组间受精率、卵裂率、优质胚胎率、临床妊娠率均无显著差异(P>0.05)。结论:在ICSI周期使用不同来源和不同数量的精子不影响其受精和妊娠结局,然而对于极度少精者实施ICSI时可增加配子在外暴露时间。Objective： To investigate the relationship of the source and amount of spermatozoa and intracytoplasmic sperm injection＇s clinical outcome in male infertility. Methods： Patients were divided into two groups according to the source of sperm（ejaculates sperm group vs epididymal aspirates and testicular sperm group）and according to the amount of sperm（severe oligozoospermia group vs exceeding oligozoospermia group）. The rates of fertilization, embryo cleavage,good quality embryo and clinical pregnancy were compared between the two group, Results： The total fertilization rate, cleavage rate, good quality embryo rate and clinical pregnancy rate was 70.8%, 95.7%, 47.1% and 35. 6%, respectively. There was not significant difference between the ejaculates sperm group and epididymal aspirates and testicular sperm group（P〉0.05） ,and between the severe oligozoospermia group and the exceeding oligozoospermia group （P〉0. 05）. But the total time to perform ICSI in the exceeding oligozoospermia group was longer than the severe oiigozoospermia group（P〈0.05）. Conclusion： There was not significant difference in fertilization and pregnancy outcome in intracytoplasmic sperm injection cycles using differrent source and amount of sperm. But it will increase the time of exposure of the gametes outside the incubator to perform the ICSI that using sperm from the exceeding oligozoospermia patients.
