摘 要:为探讨我国婴儿乙型肝炎疫苗免疫失败与a决定簇基因变异的关系,用聚合酶链反应技术(PCR)及直接测序法对27例乙型肝炎疫苗免疫失败的婴儿血清标本HBsAg的a决定簇基因进行了研究。在第419-598位核苷酸区内,与共有序列比较,14/27(51.85%)份标本有碱基变异,其中13份标本有氨基酸改变。13份中仅1份在a决定簇的第二环,为第145位精氨酸取代甘氨酸,其余12份氨基酸改变的位点在第一环或其邻近5’端,分别为129,126,122,113及110。a决定簇基因变异与转氨酶升高间无相关性。结果说明a决定簇的变异可能与乙型肝炎疫苗免疫失败有一定关系,尚待进一步研究。To study correlation between failure in hepatitis B vaccination and mutations in the "a" determinant gene by means of polymerase chain reaction and direct sequencing, we have studied "a" determinant gene in 27 infants who failed to be protected by hepatits B vaccination. Compared to the consensus sequence in the region 419-598 nt, 14/27(51.89% ) samples showed nuclcotide changes, and among them, amino-acid changes were found in 13, Except in 1 sample which had amino-acid changes (145 codon, glycine changed to arginine) in the second loop of the "a" determinant, all other 12 samples had amino-acid changes in the first loop, which were at the 129, 126, 122, 113, and 110 codons. There was no coincidence between "a" determinant gene mutation and ALT elevation. Results indicate that variant in the "a" determinant was not the major mechanism which led to failure of protection of the present hepatis B vaccination in maternal-infant transmis-
