摘 要:目的研究杭州地区中老年高脂血症患者血栓危害因子与血小板磷脂膜脂肪酸的相关性。方法对高脂血症组(81例)和正常血脂对照组(65例),用气相色谱仪分析各种甲酯化血小板磷脂膜脂肪酸的含量,用标准方法测定血液中血栓危害因子血清同型半胱氨酸(Hcy)及血栓素 B_2(TXB_2),并用多元线性回归分析二者之间的相关性。结果高脂血症患者的血小板磷脂膜脂肪酸与健康对照组间差异无统计学意义(P>0.05)。Hcy 与血小板磷脂膜廿二碳六烯酸(DHA)、n-3多不饱和脂肪酸与 n-6多不饱和脂肪酸比值(n-3 PUFA/n-6 PUFA)呈明显负相关(r=-0.277、-0.231,均P<0.01)。TXB_2与花生四烯酸呈正相关(r=0.176,P<0.05),与 DHA、廿碳五烯酸(EPA)和 n-3PUFA/n-6 PUFA 呈明显负相关(r=-0.209、-0.194、-0.238,P<0.01、0.05、0.01)。结论提高血小板磷脂膜 n-3 PUFA/n-6 PUFA 值有利于降低与血栓形成密切相关的危险因子 Hcy 和 TXB_2的生成,为选择饮食提供依据。Objective To investigate the relations of platelct phospbelipid fatty acids to thrombotic risk factors in the middle-aged and geriatric patients with hyperlipidemia in the metropolitan area of Haagzhou, Zhejiang province, Methods A questionnaire survey was conducted among 81 patients with hyperlipidemia, 50 males and 31 females, aged (57 ± 8), and 65 healthy controls, 43 males and 22 females, aged ( 58 ± 9) to collect the data about height, weight, and diet. Peripheral venous blood samples were collected to examine the total cholesterol, triglyceride, high-density lipoprotein cholesterol ( HDL-C ), low-density lipoprotein cholesterol ( LDL-C ), homocysteine ( Hcy ), 6-keto-prostaglandin F1α ( 6-kcto- PGF1α) , and thromboxane B2 ( TXB2 ) were examined by standard methods. Serum thrombotic risk factors including homocysteine and Thromboxane B2 were determined by standard methods, Platelet phespholipid fatty acids were examined by gas chromatography. The correlation between the serum thrombotic risk factors (Hcy, TXB2, and 6-keto-PG Fla) was analyzed by multivariate linear regression. Results There were no significant differences in platelet phespholipid fatty acids between the patients with hyperlipidemia and the healthy controls. Serum Hcy was significantly negatively correlated with docosahexaenoic acid (DHA) and the ratio of n-3 PUFA ( polyunsaturated fatty acids)/n-6 PUFA ( r = -0. 277 and -0. 231, both P 〈0. 01 ) . The level of semmTXB2 was significantly positively correlated with arachidonic acid ( r = 0. 176, P 〈 0. 05 ), and significantly negatively correlated with DHA ( r = - 0. 209, P 〈 0. 01 ), eicesapentaenoic acid (EPA) (r = - 0. 194, P 〈 0. 05 ), and n-3 PUFA/n-6 PUFA ( r = - 0. 238, P 〈 0. 01 ). Conclusion Increasing the ratio of n-3 PUFA/n-6 PUFA in platelet phespholipid may potentially decrease the thrombotic risks such as Hcy and TXB2 and provide a reference for diet selection.
