宣肺平喘、温阳利水法对肺心病急性加重期合并周围性水肿ADH的影响

摘　　要：目的通过多中心、随机、对照、平行、单盲临床研究,对宣肺平喘、温阳利水法治疗肺心病急性加重期合并周围性水肿疗效进行评价,并观察其对抗利尿激素(ADH)的影响。方法以肺心病急性发作期合并周围性水肿的患者为对象,随机分为两组。治疗组采用中西医结合方案治疗,对照组予以西医常规加安慰剂治疗,两组均治疗14d。比较两组疗效性指标与ADH值情况。结果治疗组在总体疗效、改善症状体征、纠正电解质紊乱、纠正酸碱失衡等方面优于对照组。两组治疗后均无明显副作用。同时治疗组在降低血清ADH水平方面优于对照组。结论针对肺心病急性发作期合并周围性水肿的中西医结合治疗可能具有提高总体疗效、改善症状体征、纠正电解质紊乱、纠正酸碱失衡优势,同时对ADH的分泌有一定的抑制作用,提示抑制ADH分泌可能是宣肺平喘、温阳利水法治疗肺心病急性加重期合并周围性水肿的部分机理。Objective： To evaluate the effectiveness of the therapy Dispersing Lung- Qi to Relieve Asthma, Warming Yang to Promote Diuresis on the level of ADH of patients during acute agrravate stage combined with peripheral hydrops, through multi- center, randomly, comparative and parallel clinical experiments. Methods： Patients were randomly divided into the treatment group （received the comprehensive treatment of TCM and WM） and the control group（received the conventional treatment of WM and placebo）. Patients in both groups were treated for 14 days. Indexes association with the therapentic effect and the level of ADH were observed. Results： The effects of the treatment group was better than of the control group on the general curative effect, reducing integral of symptoms and sirens, rectifying electrolyte and acid - alkaline - balance disturbances, etc. The safty analysis of the two groups has no significant abnormality. Meanwhile, the treatment group have more superiority than the control group on reducing serumal ADH. Conclusion： The elementary research indicates that the the Comprehensive Treatment of TCM with WM probably has more advantages in increasing the general effect, improving symptoms and sirens, rectifying electrolyte and acid - alkaline - balance disturbances, etc. The therapeutic method can restrain secretion of ADH. It indicates that restraining secretion of ADH maybe part of the mechanism of the therapy Dispersing the Lung- Qi to Relieve Aasthma, Warming Yang to Promte Diuresis to treat cor pulmonale patients during acute agrravate stage combined with peripheral hydrops.
