摘  要:目的探讨早期应用重组人类红细胞生成素(rHuEPO)和铁剂,后期长期间隔补铁防治早产儿贫血的疗效,及对早产儿生长发育、智能方面的影响。方法2002.2~2005.2入住本院NICU的早产儿共179例.随机分为治疗组64例、对照组57例、空白组58例,治疗组应用rHuEPO(生后第1周开始,750IU·kg^-1·w^-1,共6w)及铁剂(生后14d开始,元素铁2mg·kg^-1·d^-1,共6w),以后每周补铁1次(元素铁2mg·kg^-1),直至12月龄。对照组生后应用rHuEPO6w及铁剂6w同治疗组,方法同治疗组,不继续补铁。空白组不用rHuEPO及铁剂治疗。观察3组早产儿HGB、血清铁蛋白(SF)、体重、身高动态变化至12月龄,6、12月龄进行DDST智能筛查。结果治疗组、对照组12w内HGB较空白组明显升高(P〈0.01),4-12月龄治疗纽HGB较对照组、空白组明显升高(P〈0.01),3-12月龄治疗组SF较对照组、空白组明显升高(P〈0.01),6-12月龄治疗组体重、身高较对照组、空白组明显增长(P〈0.01、P〈0.05)。6、12月龄DDST治疗组较对照组、空白组明显优胜(P〈0.05)。结论早产儿生后早期应用rHuEPO和铁剂可以减轻早产儿早期贫血,继续长期间隔补铁,可以有效防治早产儿后期贫血,改善生长发育。Objective: To explore the curative effect treated with erythropoietin and iron in early stage and long - term interval iron in later stage for the prematurity anemia. The treatment affected grow and intelligence was explored. Methods: 179 preterm infants were randomly assigned to treated group (64 eases) or contrasted group (57 cases) or blank group (58 cases). The treated group were given rHuEPO (750·kg^-1·w^-1, for 6 weeks) from the first week of life and elemental iron (2mg·kg^-1·d^-1, for 6 weeks) from the fourteenth day of life, They were given long - term interval iron after above treatment to 12 months old. The contrasted group were given rHuEPO for 6 weeks and iron for 6 weeks, They weren't given iron continue. The blank group weren't given rHuEPO and iron. Three groups were observed to 12 months old concerning HGB, SF, weight, height, intelligence. Results: The hemoglobin was significantly raised than the blank group in the treated group and the contrasted group for 12 weeks old ( P 〈0. 01 ). The hemoglobin was significantly raised in the treated group from 4 to 12 months old ( P 〈0. 01 ). Serum ferritin was significantly raised in the treated group from 3 to 12 months old ( P 〈0. 01). The weight and height was significantly raised in the treated group from 6 to 12 months old ( P 〈 0. 01, P 〈 0. 05). The intelligence with DDST was significantly superior in the treated group ( P 〈 0. 05). Conclusion : It can lighten prematurity early stage anemia that rHuEPO and iron is given early. It is efficacious concerning long - term interval iron for prematurity later stage anemia and grow.
