保脾术治疗外伤性脾破裂26例分析

摘　　要：目的探讨外伤性脾破裂的不同保脾方法。方法1998年10月～2005年6月收治的26例脾破裂采用各种保脾术治疗,取得成功。其中2例采用保守治疗,9例行脾修补术,10例行脾部分切除术,5例行脾切除脾片移植术。结果26例全部获得成功,无严重并发症发生,术后3个月～6个月,患者白细胞、血小板、IgG、IgM和IgA数值均恢复正常。结论根据脾损伤程度分级标准选择适当的保脾方法,能获得良好的效果。Objective To explore the application of variable spleen-conserving surgeries for splenic injury. Methods From Oct. 1998 to Jan.2005,26 patients with splenic injury received spleen-conserving surgeries successfully. Among them, 2 cases received conservative management,9 underwent splenorrhaphy, 10 underwent hemispleneetomy, and 5 underwent splenectomy and spleen fragment transplantation. Results All 26 patients recovered well without complications. The white blood cells, platelet, IgG, IgM and IgA values were in normal ranges 3 and 6 months postoperatively. Conclusion Suitable spleen-conserving surgeries should be selected according to splenic injury grade. The procedure was feasible and the effect was nice.
