骶会阴肛门成形术治疗先天性肛门直肠畸形的临床观察

摘　　要：目的探讨骶会阴肛门成形术治疗先天性肛门直肠畸形的手术效果。方法总结1998-2006年采用骶会阴肛门成形术治疗直肠阴道瘘6例,直肠膀胱瘘3例,直肠尿道瘘2例的临床资料。结果手术时间平均125 min。术后2-4 d内恢复肠道功能,平均住院时间12 d。术后无1例瘘复发及肛门失禁、肛门狭窄,11例患者均获痊愈。结论经骶会阴肛门成形术能够充分显露瘘管和保护肛门外括约肌系统,重建的直肠肛门更接近于生理,大大减少肛门失禁等并发症,复发率低,近期疗效好。Objective To evaluate the effect of sacroperineal anoplasty to treat congenital rnaiformations of the anus and rectum. Methods The clinical data of 11 cases treated with saeroperineal anoplasty, including 6 cases of rectovaginal fistula, 3 cases of rectovesical fistula, 2 cases of rectourethral fistula was summarized. Results The average operation duration was 125 minutes. The function of intestinal tract recovered with 2 - 4 days, and average hospitalization duration was 12 days. All 11 patients recovered without any relapse of fistula, anal incontinence and anal stenosis. Conclusion Sacroperineal anoplasty can thoroughly expose fistula and protect external anal sphincter with reconstructed anorectum approximating to normal, low complications of anal incontinence,low relapse rate and better short-term effect in treating congenital malformations of the anus and rectum.
