输卵管通液与子宫内膜异位症发病关系的探讨

摘　　要：目的：通过观察有输卵管通液史的不孕者中子宫内膜异位症（endometriosis，EM）的发生情况，探讨输卵管通液是否是EM的高危因素。方法：将1994年1月至2005年1月，因不孕在奉院行腹腔镜检查的患者作为研究对象，共7563倒。原发不孕3355例，继发不孕4208例。根据镜下所见输卵管通畅情况将患者分为2组，输卵管通畅组3704例，闭塞组3859倒；根据患者输卵管通液次数，将每组又分为4小组（无、1-5次、6～10次、10次以上）。结果：①输卵管通畅组的EM发生率45．5％，明显高于闭塞组（2．0％），两组比较差异有统计学意义（P〈0．05）。结果表明输卵管通畅情况与EM有关系。②输卵管通畅组的4小组EM的发生率分别20．0％、47．8％、59．0％、71．1％。组间比较差异有统计学意义（P〈0．05）。显示做过通枢的患者EM发生率高于未做过通水者。通水次数越多EM发生率越高，，而在闭塞组中无差异。③在原友不孕患者中EM的发生率（36．3％），高于继发不孕者（20．9％），差异有统计学意义（P〈0．01）。结论：输卵管通液术可能是不孕患者发生EM的高危因素。Objective：To investigate the ineidenee of endometriosis（EM） in the infertility patients with history of hydrotuhation so as to explore the correlation between hydrotubation and EM. Methods： A Iotal of 7563 eases of infertility entered this study from January 1994 to January 2004, admitted in our hospital for laparoseopy. The numbers of primary infertile and seeondary infertile cases were respectively 3 355 and 4 208. All the cases were divided into two groups：the unobstructed was 3 704, the obstructed was 3 859; According to times of hydrotubation each group was further divided into four subgroups： 0. 1 - 5, 6 - 10 and 〉 10. Results： The rate of EM in the unobstructed group （45, 5% ） was prominently higer than that of the obstructed （2.0%） （P 〈 0. 05） , suggesting that the fallopian tube ohstruction was closely correlated to EM, The rate of EM in the four subgroups of unobstnlcted was respectively 20. 0% ,47.8% ,59.0% ,71. 1% （ P〈 0. 05 ）, which suggested EM was likely to be more common in hydrotubated cases, and the rate of EM increased with times of hydrotubation. There was no diffcrcncc among the subgroups of the obstructed, Among the primary infertility EM incidence （36.6%） was higher than that of the secondary infertility （20. 9% ） （ P 〈 0.01 ）. Conclusion： Fallopian tlydrotubation is a high risk factor for endometriosis,
