中枢兴奋剂莫达非尼对认知功能的影响及可能机制

摘　　要：新型的中枢促醒剂莫达非尼主要作用是治疗发作性嗜睡症和自发性嗜睡症,以及阻塞型睡眠呼吸暂停综合征和倒班工作所致的睡眠紊乱等症的过量日间睡眠,与传统中枢兴奋剂相比,能够在调节睡眠、促进觉醒的同时减少各种依赖、戒断、动作不稳等不良反应的产生。目前有研究表明治疗剂量的莫达非尼还对许多原因引起的认知功能障碍有明显改善作用,但其作用机制目前尚不清楚,推测其对认知的影响可能与特定脑区中递质释放及能量代谢改变有关,由此猜测脑中调节递质释放的某些蛋白物质是否也会受莫达非尼的影响,但是莫达非尼表现出来的对认知障碍的改善作用,可能为此类疾病的治疗提供了新线索。Modafinil is a novel wake-promoting compound for improving wakefulness in patients with excessive daytime sleepiness associated with narcolepsy, obstructive sleep apnea/hypopnea syndrome, and sleep disorders induced by shifting work period. Comparing with other central stimulant, modafinil possesses less adverse reactions such as addiction, action unsteadiness and abstinent symptom. In addition to its clinical indications, recent studies have shown that modafinil is effective in maintaining cognitive performance and subjective ratings of mood. But the mechanism is still unclear probably due to neurotransmitter release of certain protein related substances and energy metabolism in special some encephalic regions during cognition improvement. These effects of modafinil may provide a new clue for the therapeutic measure of cognitive disorders.
