摘　　要：目的对S—ROM型全髋关节置换术（THA）治疗成人发育性髋关节结构不良（DDH）继发骨关节炎（0A）的近期疗效进行评价。方法对成人DDH继发OA的12例（15髋）患者进行S—ROM型THA。对CroweⅢ型和Ⅳ型患者应用小直径髋臼假体和小号细直柄股骨假体。结果12例均一期愈合，除3例CroweⅣ型患者出现暂时性腓总神经麻痹（2～4个月后恢复）、2例脱位外，余均顺利康复。Harris评分由术前的40．18分恢复到术后的85．34分。结论DDH患者存在骨性和肌性结构异常，手术难度较大；S—ROM型假体适合对DDH的治疗，近期疗效显著。[Objective] To evaluate the peformance of the S-ROM total hip system osteoarthritis （OA） secondary to developmental dysplasia of hip （DDH） in the short- term. [Methods] 15 total hip arthroplasties （THA） in 12 patients with OA secondary to DDH were performed with S-ROM total hip system. Among patients with Crowe type-Ⅲ and Crowe type-Ⅳ, the acetabular reconstruction was done with the small acetabular components and small DDH stems. [Results] The 12 cases healed smoothly. There were 3 postoperative temporary common peroneal palsy resolving within 2-4 months of 3 in Crowe type-Ⅳ, and 2 postoperative dislocation, others were recovered successfully. The average Harris score was increased from 40. 18 to 85.34 points after the surgery. [Conclusion] It is a challenging work to the orthopatedic surgery because anatomical abnormality of bones and parenchyma is commonly seen. And patients with DDH are ideal indications for using the S-ROM total hip system. Clinical results of short-term can be well expected.
