鼻内镜下下鼻甲改良成形术治疗下鼻甲肥大(附36例报告)

摘　　要：目的比较两种治疗下鼻甲肥大的手术方式,并评价其疗效。方法下鼻甲肥大患者70例,其中行鼻内镜下下鼻甲改良成形术36例(72侧,实验组);下鼻甲部分切除术34例(68侧,对照组)。手术前后分别行鼻阻力测定,记录两组术后创面愈合、下鼻甲位置、鼻塞改善情况。结果①鼻阻力:两组术前及术后6个月比较无差异(P>0.05);术后12个月时两组差异显著(P<0.05);术后6、12个月时与术前比较均有显著性差异(P均<0.01)。②疗效:实验组创面正常愈合,干痂存留7 d,平均愈合时间7 d;对照组创面为瘢痕愈合,干痂存留20 d,平均愈合时间13 d。实验组术后无一侧粘连增生,对照组有9侧粘连。实验组鼻甲形态、位置适当66侧,鼻塞完全解除35例;对照组分别为22侧,27例。结论鼻内镜下下鼻甲改良成形术是治疗下鼻甲肥大的较理想的术式,符合鼻腔正常生理功能及微创原则。[Objective] To evaluate the effects of transnasal endoscopic improved urbinoplasty on the inferior turblnate hypertrophy. [Methods]70 cases with the inferior turbinate hypertrophy were randomly assigned into two groups, 36 cases （72 sides） treated by transnasal endoscopic improved urbinoplasty , 34 cases （68sides） treated by partial inferior turbinectomy. All cases were followed up 6 to 12 months. Nasal resistance were determined before and the position of nosepiece,amelioration the condition of nhinocleisis,the condition of wound in the two groups were recorded after the two operations. [Results] Nasal resistance of the two groups were no significantly different （ P 〉0. 05） preoperation and 6 months postopertion ,and there were significantly different in 12 months postopersion （ P〈 0. 01）. The normal crue time in test group was 7 days and the cicatrice crue time in control group was 13 days. Rhinoscab existed for 7 days in test group and 20 days in control group. 66 sides had the normal position of nosepiece in test group and 22 sides in control group. Nhinocleisis disappeared completely in 35 cases of test group and 27 cases of control group. [Conclusion] Transnasal endoscopic improved urbinoplasty is considered as an effective method to treat the inferior turbinate hypertrophy,it can keep the normal functionof nasal cavity and coin cide microtrauma principle.
