摘  要:目的:观察乌司他丁(UTI)、缺血预处理(IPC)对肺缺血-再灌注(I/R)损伤的保护效果;探讨二者联合对肺I/R损伤的保护作用加强的原理。方法:将24只新西兰大白兔不拘雌雄随机分为4组,每组6只(Control组,IPC组,UTI组,UTI+IPC组)。分别在缺血前、缺血60min、再灌注30min及120min抽动脉血测血清超氧化物岐化酶(SOD)、丙二醛(MDA)、一氧化氮(NO);取支气管肺泡灌洗液(BALF)测肺通透指数(LPI);取BALF离心沉渣显微镜下计数白细胞数量,中性粒细胞(PMN)分类;取肺组织测肺组织湿/干比(W/D),制成匀浆测肺组织髓过氧化物酶(MPO),在光镜下观察各实验组肺组织结构变化,观察肺组织损伤程度(LTD)。结果:与Control组相比IPC组、UTI组的W/D、LPI、BALF中白细胞数及中性粒细胞(PMN)数、MDA及MPO含量均显著降低(P<0.01),血清SOD和NO含量显著升高(P<0.01),肺组织损伤程度(LTD)明显减轻,联合使用UTI与IPC效果更明显。结论:中性粒细胞在肺内聚集、氧自由基生成增多和内源性NO减少是引起肺I/R损伤的重要因素;IPC和UTI均对肺组织结构有明显的保护作用,二者联合效果更明显。Objective To observe the conservation effective and mechanism of ulinastain and ischemic preconditioning to lung that received Isehemia/Reperfusion , and the protection mechanism to lung [schemia/Reperfusion injury of ulinastatin combining with ischemic preconditioning. Methods 24 aduh healthy New Zealand rabbits of either sex were randomly divided into four equal groups,6 rabbits in each group(Control,IPC , UTI , UTI + IPC) . To draw the arterial blood at four time(before ischemic, ischemic fore 60 minuts ,30 minuts of reperfusion, 120 minuts of reperfusion), to detect serum levels of SOD , MDA and NO ; detect LPI , neutrophils percentage in BALF, W / D andMPO in lung tissue . To examine the LTD by view of the histological appearances of lung under light microscope. Results Conferred to Control group the W/D, LPI, quantity of WBC in BALF,MDA and MPO activity were significantly lower in IPC group , UTI group( all P 〈0.01, respectively ), SOD and NO are significantly higher than those in Congtrol group. ( all P 〈 0.01, respectively ). tissues demonstrated the degree of lung injury been alleviated significantly. The protective effect in UTI + IPC group is more singnificantly, than in UTI and IPC group. Conclusion The neutrophil been collected up in lung , more oxyradical generation and the decrease of NO are the important factors in making the lung Ischemic Reperfusion surgery. Both IPC and UTI have important protection to the lung tissues. , the protective effect is more significantly by UTI combining with IPC .
