重型肝炎应用人工肝支持治疗中并发症的防治

摘　　要：目的研究重型肝炎进行人工肝支持治疗血浆置换时并发症的发生及其防治方法。方法分析57例重型肝炎患者采用血浆置换175次治疗过程中出现并发症49次的临床资料。结果并发症总发生率28．00％（49／175），其中血浆过敏反应21．14％（37／175）、凝血3．43％（6／175）、低血压2．86％（5／175）、出血0．57％（1／175）、无继发感染发生。经及时处理后大都缓解，无严重影响患者健康的并发症。结论积极防治并发症对成功完成治疗是重要的，血浆置换对重型肝炎的治疗是安全可行的。Objective To study prevention and treatment on complications of plasma exchange （ PE ） in treating patients with severe hepatitis. Methods The clinical data of 49 times of complication occurring in 57 cases with severe hepatitis who received 175 times PE therapy were analyzed. Results The occurrence rate of complications was 28.00% （49/175 ） ,including 21.14% （ 37/175） of plasma allergy,3.43% （ 6/175 ） coagulation,2.86% （5/175） hypotension and 0.57% （ 1/175 ） bleeding. The secondary infection was not found. Of all patients, most were relieved after treatment. No severe complications were found. Conclusions Active prevention and treatment of the complications is of importance to successful completion of treatment of severe hepatitis. Plasma exchange is proved to be safe in treating severe hepatitis.
