摘　　要：目的:探讨修复圆形或椭圆形皮肤缺损的有效方法。方法:在创面一侧的切线上设计一舌形皮瓣,使其成为一不对称Z形皮瓣,即一个皮瓣呈三角形,另一个皮瓣呈舌形。在深筋膜表面(面部在脂肪浅层)同时掀起三角形及舌形两个皮瓣,仔细止血,最后将两皮瓣相互交错,间断缝合固定,进行转移修复创面。结果:105例皮瓣修复病例中,除有3例由于缝合张力稍大三角形皮瓣远端创缘表皮部分坏死外,余102处皮瓣均Ⅰ期愈合,全部病例均按时拆线,坏死表皮在伤后12天左右愈合,对修复效果无明显影响。病变切除后创面最小者0.7cm×0.7cm,最大者6.0cm×6.0cm。全部患者术后未见明显局部或五官变形,外观良好,效果满意。结论:改良Z形皮瓣是修复圆形或椭圆形皮肤缺损的有效方法。Objective To investigate a effective method to repair the round or ellipse skin defects. Methods To design a tongue shape flap at the tangent of the skin defect,make it in a anisomerous Z form. In this way, one of the skin flap is a triangle,the other is in a tongue form.Pull the two kinds of flaps up on the surface of the deep fascia, stop bleeding, and then change the position of the two flaps ,suture them interrupted. Results Only 3 of the 105 cases receiving this kind of flap repair suffer partial epidermis necrosis on the distal end of the wound because of hyper tension, the other 102 cases enjoy primary healing. All the cases are sutured out on time. Cases with epidermis necrosis get wound healing in 12 days.there is no negative influence to the reparing result.The minimum raw surface after removing the damaged tissue is 0.7cm×0.7cm, while the maxmum is 6.0cm×6.0cm. None of the patients have obvious local or whole facial disfiguration.Good appearance and satisfaction is obtained by this method. Conclusion Improved Z shape flap is a effective way of reparing round and ellipse skin defects.
