Correlation between atrial natriuretic peptide and renin-angiotensin-aldosterone system in congestive heart failure

摘  要:目的研究慢性充血性心力衰竭病人心房利钠肽(atrial natriuretic peptide,ANP)与肾素-血管紧张素-醛固酮系统的关系。方法采用放射免疫分析法测定对受试者血浆血管紧张素Ⅱ(angiotensinⅡ,ATⅡ)、醛固酮(aldosterone,ALD)及ANP含量。结果心力衰竭组血浆ANP、ATⅡ、ALD显著高于对照组,治疗后血浆水平均下降,与治疗前差异有统计学意义(P<0.001)。ANP水平与ATⅡ水平呈正相关(P<0.01),与ALD水平呈正相关、(P<0.05)。结论同步监测ATⅡ、ALD、ANP有助于早期发现心力衰竭,判断病情及预后。Objectives To investigate the relationship between atrial natriurtic factor (ANP) and reninangiotensin-aldosterone (RAA) system in patients with congestive heart failure (CHF). Methods The plasma levels of angiotensin Ⅱ (ATⅡ) aldosterone (ALD) and ANP were tested by special radio-immunoassay. Results The plasma levels of ANP, AT Ⅱ , ALD in group of CHF were significantly higher than in the control group. The plasma levels of ANP, AT Ⅱ , ALD in group of CHF after therapy were descended and markedly differed from that before therapy (P〈0.001). ANP were positively correlated with AT Ⅱ (P〈0.01) and ALD (P〈 0.05) in CHF. Conclusions The comparative insufficiency of ANP in circulating blood, or the enhancement of RAA system's activity may be one of the pathogenesis of heart failure. Dynamic observation of changes of AT Ⅱ , ALD, ANP may be very useful in judgement of severity and prognosis of patients with CHF.
