Zweymuller螺旋臼在高龄全髋关节置换术中的应用

摘　　要：目的评价Zweymuller螺旋臼应用于高龄病例全髋关节置换术(THR)的临床疗效。方法对20例年龄在75岁以上的高龄髋部疾患病例施行THR,其中骨性关节炎4例,股骨颈骨折14例(GardenⅣ),髋关节骨性强直和THR术后假体松动各1例。所有病例髋臼侧采用Plus公司双锥面螺旋臼;股骨侧2例采用骨水泥柄,其余病例采用Zweymuller柄。术后随访42～66个月。术前Harris评分18～62分。结果术后所有患者髋关节疼痛消失,行走能力增强,临床和X线显示假体位置满意,无松动迹象,Harris评分75～95分。结论双锥面螺旋臼作为生物型假体应用于高龄THR术病例,短期随访效果满意。Objective To evaluate the clinical outcome of Zweymuller cementless bicon cup used in THR in patients beyond 75 years old. Methods Twenty total hip replacernents（THR） were performed in 4 patients with osteoarthritis, 14 with femur neck fracture （Garden Ⅳ）, 1 with fusing hip and 1 with loosed prosthesis after THR. The Zweymuller cementless biocon cup was used in all patients,and Zweymuller cementless femoral stems were used in 18 patients and cementing femur stems for the others. The follow-up time ranged from 42 to 66 months（average, 52 months）. The preoperative Harris hip score ranged from 18 to 62（average,45）. Results The pain was completely relieved and the ability of walk was strengthened in all patients after operation. At the lastest followup, clinical and radiographic analysis demonstrated that the positions of stems and cups were unchanged and all showed radiographic evidence of bone ingrowth, and Harris hip score ranged from 75 to 95 （average,90）. Condusion As a bio-prosthesis, the cementless biocon cup used in THR in the elderly can provide stable fixation.
