摘 要:目的了解甲状腺机能亢进患者治疗前骨代谢生化指标、骨密度(BMD)的变化,观察甲状腺机能亢进患者治疗后甲状腺功能恢复正常时骨代谢情况。方法选择未治疗的Graves痛(GD)患者27例(治疗组),采用双能X线吸收法(DEXA)测定其治疗前后BMD、血钙(Ca)、磷(P)、碱性磷酸酶(ALP)、血清骨钙素(BGP)、1型胶原交联羧基末端肽(1CTP)、1型前胶原羧基末端前肽(P1CP)、甲状旁腺激素(PTH)。并选择与患者年龄、性别和女性月经状态相匹配的健康志愿者30例作为对照组。结果GD患者治疗前ALP、BGP、1CTP、P1CP显著高于治疗后及对照组,BMD于治疗前显著低于治疗后及正常对照组,治疗组治疗前后不同部位BMD的测定值分别为,腰椎L2~L4(1.027±0.094)g/cm^2VS(1.140±0.072)g/cm^2,股骨颈(0.803±0.161)g/cm^2VS(0.929±0.127)g/cm^2,Ward三角(0.669±0.088)g/cm^2VS(0.742±0.112)g/cm^2,股骨粗隆(0.698±0.113)g/cm^2 VS(0.774)±0.097)g/cm^2。血Ca、P、PTH治疗前后及与对照组之间差异无统计学意义。结论甲状腺机能亢进治疗前破骨细胞功能增强,BMD癌低.两时律右威晋细曲.活桂增加.甲波腹功能恢童正常后.晋矿会号增加.Objective To study the changes in bone metabolism biochemical indexes and bone mineral density (BMD) in the patients with Graves disease(GD) before and after treatment. Methods BMD, blood Ca, P, alkaline phosphates(ALP), bone glia protein (BGP), carboxy terminal propeptide of type Ⅰ procollagen(P1CP), carboxy terminal cross-linked telopeptide of type Ⅰ collagen(1CTP) ,parathyroid hormone(PTH) were comparatively studied before and after treatment in 27 patients with GD(treatment group) and the control group. Results Blood ALP,BGP, P1CP, 1CTP of GD patients before treatment were higher than those after treatment and of the control group, and BMD was lower in GD patients before treatment. Different BMD values in treatment group before and after treatment were as follows,lumbar vertebrae L2 -L4 (1. 027±0. 094) g/cm^2 vs (1. 140±0. 072) g/cm^2 ,femoral neck (0. 803±0. 161) g/ cm^2 vs (0. 929±0. 127) g/cm^2 ,Ward triangle (0. 669±0. 088) g/cm^2 vs (0. 742±0. 112) g/cm^2 ,femoral trochanter (0. 698±0. 113) g/cm^2 vs (0. 774)±0. 097) g/cm^2. Blood Ca,P,PTH had no significant difference before and after treatment as well as between two groups. Conclusion BMD shows reduction in hyperthyroidism, but when the function of thyroid gland in GD patients after treatment returns to normal,their BMD amount increases.
