MRI随访观察对非前列腺癌患者的诊断价值

摘　　要：目的 探讨MRI随访观察对非前列腺癌患者的诊断价值。方法 对1992—05-2004—09在北京大学第一医院进行过2次及2次以上MR检查（MR复查与初次MR检查间隔时间1．5—97个月），并经病理或临床诊断为非前列腺癌的25位患者的临床资料和MR表现进行回顾性分析。比较初次和复查结果。结果 25例非前列腺癌患者初次MR检查时，前列腺体积（114．63±14．48）mm^3，前列腺内发现异常信号16例，外周带15例、中央腺体1例。复查时，前列腺体积（103、71±13．34）mm^3，前列腺内发现异常信号15例，外周带14例、中央腺体1例。复查MR检查时16例定性诊断无变化；9例有变化。以上结果经统计学处理，除患者年龄初诊和复查时差异有统计学意义（P=0．00）外，其他差异均无统计学意义（P〉0．05）。结论 经确诊或临床诊断为非前列腺癌患者，常规MR随访常不能提供有价值的信息。Objective To evaluate the value of MRI follow - up in patients with non - tumoral lesions in prostatic gland. Methods Clinical and MRI data in 25 patients with non - tumoral diseases（ mainly BPH and/or prostatitis） confirmed pathologically and undergone two or more MR examinations （ the interval between the first and next examination was 1,5 -97 months ） for follow - up were retrospectively analysed . MR findings of the first and second examination were comparatively analyzed . Results The volume of the prostate was （ 114.63±14.48） mm^3 and （ 103.71± 13.34） mm^3 .and the abnormal signal of the prostate was detected in 16 cases（ 15 in peripheral zone and 1 in central gland） and 15 cases （14 in peripheral zone and 1 in central zone） in initial and following MR examination respectively. The qualitative diagnosis was the same as before in 16 patients and changed in 9 patients in repeated MR examination. The results showed that there was no statistically difference between the two times of MR examination （P 〉 0.05 ） except the patient＇ s age （ P = 0.00 ）. Conclusion Follow - up of patients with non - tumoral lesions in prostate gland with MR imaging usually does not give useful informations in addition to the first MR examination.
