复方健骨关节汤治疗骨关节炎的实验研究

摘　　要：目的:观察复方健骨关节汤对骨关节炎(OA)的治疗效果并探讨其作用机制。方法:将24只中国白兔随机分为正常对照组(A组)、空白对照组(B组)、中药治疗组(C组),每组各8只,A组不造模,B、C组均采用Hulth法建立兔OA模型;4w后,取胫骨平台关节软骨和滑膜进行光镜观察,采用免疫组化和末端原位标记细胞凋亡检测法分别检测关节软骨和滑膜中的白细胞介素-1β(IL-β),基质金属蛋白酶-1、3(MMP-1、3),软骨细胞凋亡指数(AI)的水平。结果:B组IL-1β,AI,MMP-1、3的水平均非常显著高于A组(P<0.01);B组IL-1β,MMP-3水平显著高于C组(0.01<P<0.05),而AI和MMP-1水平则非常显著高于C组(P<0.01)。光镜观察,B组退变较其余2组明显,C组退变较A组明显。结论:复方健骨关节汤能减轻OA软骨细胞的凋亡,抑制IL-1β,MMP-1、3的产生,具有抗炎、保护和修复关节软骨的作用,从而对OA有一定的治疗作用。Objective：To observe the effect and study the mechanism of FuFangJianGuGuanJie-Tang （FJT） in the treatment of Osteoarthritis {OA）. Methods：Twenty-four Chinese rabbits were randomly divided into normal group （A group）, test group （B group）, traditional medicine group （C group）. There were 8 rabbits every group. The animals of B,C groups all were choosed to produce model of OA by the Hulth method except for A group. After 4 weeks, the specimens were procured from articular cartilage and synovium for medial plateau of tibia for light microscopic, immunohistochemical method and in situ end labbelling method. They were used to detect the degeneration changes of cartilage and test the level of interleubin- 1β（ IL- 1β）, matrix metalloproteinase- 1,3 （ MMP- 1,3 ） and apoptotie index（AI）. Results： The amount of IL- 1β, MMP-1,3 and AI in B group was highest than that in A group （P〈0. 01 ）. The amount of IL-1β and MMP 3 in B group was higher than that in C group（0.01〈P〈0. 05）, but the amount of AI and MMP- 1 was highest than that in C group（P〈0.01）. From light microscopic, The degenerative changes of cartilage in B group were worse than that in A,C groups, the degenerative changes of cartilage in C groups was worse than that in A group. Conclusions：The study indicated that FJT could lighten the apoptosis of Chondrocytes, resist the producing of IL-1β and MMP-1,3 in OA and has some effect of eliminating inflammation, protesting and plerosising cartilage. Therefore, FJT had some remediable action to OA.
