腰椎管减压椎弓根螺钉内固定治疗腰椎管狭窄症疗效观察

摘　　要：目的 探讨腰椎管狭窄症的手术减压指征、减压方法及椎弓根螺钉内固定方法的治疗效果。方法 腰椎管狭窄症患者23例，采用广泛椎板切除减压、椎间关节融合、横突间融合及椎弓根螺钉内固定治疗。结果 随访6～36个月，术后优良率87．0％，发生内固定松动断裂1例。结论 腰椎管减压和脊柱固定可以改善腰椎管狭窄临床症状，术中减压彻底以及获得术后植骨部位的骨性融合是保证治疗效果的关健。Objectives To assess surgical treatment of lumbar spinal stenosis and the principle and significance of transpedicle internal fixation in the decompression of lumbar spinal stenosis. Methods Twenty-three cases of lumbar spinal stenosis, in whom there were 15 men and 8 women, aged 35.7 years and the average history of 4. 2 years, underwent extensive vertebral lamina decompression, transvertebral pedicle intermal fixation, intervertebral fusion and intertransveme process bone graft. Restilts The patients were followed up from 6 to 36 months and averaged 17. 5 months. The excellent and good rates were 87.2%. Internal fixation loose and breakage occurred only in one case. Conelusion Surgical decompression and internal fixation can mitigate the symptoms of lumbar spinal stenosis. Complete decompression and bone grafts fusion are essential for achieving a successfnl effect.
