摘　　要：目的探讨可吸收球囊椎体成形术治疗胸腰椎爆裂骨折的可行性。方法对16例胸腰椎爆裂骨折患者，在经椎弓根螺钉系统撑开复位固定的基础上，通过自制的配套器械经伤椎椎弓根向上推顶终板，使伤椎椎体内囊腔扩大成形，将采用可吸收高分子材料-DL-乳酸与ε-己内酯（70：30）的共聚物（PDLLA—CL）制作的可吸收球囊，置于椎体内，注入自固化硫酸钙骨水泥，行可吸收球囊椎体成形术。结果切口均Ⅰ期愈合，无异物反应，无神经症状加重或出现新的神经损伤症状，术后摄片及CT复查，无渗漏发生。术后随访时疗效均满意，未发现内固定失败、纠正度丢失及伤椎塌陷加重。结论可吸收球囊椎体成形术治疗胸腰椎爆裂骨折，可恢复上终板高度，有效修复经椎弓根螺钉系统撑开复位后伤椎椎体内形成的空隙，可从根本上防止骨水泥渗漏所引起的相关并发症的发生，为临床治疗胸腰椎爆裂骨折提供了一种新的方法。Objective To study the feasibility of absorbable balloon vertebroplasty for the treatment of thoracolumbar burst fractures. Methods Sixteen patients with fresh thoracolumbar burst fractures were treated with absorbable balloon vertebroplasty with pedicle screw system. The defects in the injured vertebral body were enlarged through rasing the upper end plate transpedicularly with the self-made instrument, then absorbable balloon vertebroplasty was performed by putting the absorbable balloon into the vertebral body transpedicularly, and calcium phosphate cement （CPC） was injected into the balloon. Results All cuts were healed primarily after operation without leakage, general and local reaction, ag- gravation of nerve symptom or new injury symptom. All patients were recovered without implant failure or correction loss. Conclusion Absorbable balloon vertebroplasty for the treatment of thoracolumbar burst fractures can restore the height of the upper end plate and intracorpereal porosity, hence, complications caused by cement leaking are fundamentally solved. It provides a new method for clinical treatment of thoracolumbar burst fractures.
