枕部经小脑幕入路切除儿童小脑前上部肿瘤（附八例报告）

摘　　要：目的研究和探索切除儿童小脑前上部肿瘤的手术方法。方法枕部经小脑幕入路切除儿童小脑前上部肿瘤8例。结果全切除7例，大部分切除1例，无手术死亡。结论枕部经小脑幕入路适用于小脑上蚓部或小脑半球上部靠近小脑幕的儿童后颅窝肿瘤切除。该入路手术损伤小，术野开阔，肿瘤暴露良好，操作简单，肿瘤全切除率高，术后并发症少，效果满意。Objective To study the occipital transtentorial approach in removal of the anterosuperior cerebellar tumors in children. Methods 8 cases of anterosuperior cerebellar tumors in children were resected via the occipital transtentorial approach. Results Of the 8 cases, total removal was achieved in 7 cases and gross total removal in one case. There was no death. Conclusions The occipital transtentorial approach can extend the wide operative view to the anterosuperior portion of cerebellum. By this approach the rate of total removal is high and the postoperative complication is very low. It is suggested that the occipital transtentorial approach is safe, simple and very useful for removing superior vermis or anterosuperior cerebellum close to the tentorium tumors.
