氯沙坦对糖尿病肾病细胞间粘附分子-1的影响

摘　　要：目的观察氯沙坦对早期糖尿病肾病(IDN)患者血清、尿液细胞间粘附分子-1(ICAM-1)水平的影响。方法48例IDN(不伴高血压)患者随机分为两组,一组仅降血糖治疗,另一组联合服用氯沙坦50 mg/d,共观察4周;应用酶联免疫吸附试验(ELISA)方法检测IDN患者治疗前后血清、尿液ICAM-1水平,并分别与健康对照组20例进行比较。结果IDN患者血清、尿液ICAM-1水平均明显高于对照组(P<0.01),氯沙坦治疗后,血清ICAM-1水平、尿液ICAM-1/Cr均显著下降(P<0.05);而未服用氯沙坦组无明显变化。结论氯沙坦治疗早期DN有效,该药可能通过减少ICAM-1的产生抑制炎症反应而保护肾脏。Objective To observe the effects of Losartan on the level of serum and urinary intercellular adhesion molecule - 1 （ICAM - 1 ） in patients with incipient Diabetic nephropathy （IDN）. Methods 48 patients with IDN （ without hypertension） were randomly divided into 2 groups, one group took Losartan 50 mg daily with the treatment of blood glucose lowering for 4 weeks,and the other only with blood glucose lowering;Serum and urinary ICAM - 1 concentrations were measured by enzyme linked immunosorbent assay （ELISA） before and after the treatment. Results The levels of serum ICAM - 1 and urinary ICAM - l/Cr （creatinine） were significantly higher in cases with IDN than that of control group （ P 〈 0, 01 ）, and significantly decreased after taking Losartan （P 〈 0.05 ）, but showing no difference in the group without taking Losartan. Conclusions Losartan is effective to protect kidney for IDN patients as an anti - inflammatory drug via inhibiting the expression of ICAM - 1.
