清热化湿活血法治疗原发性痛风合并慢性肾损害(湿热血瘀证)的临床研究

摘　　要：目的:观察清热化湿活血法治疗原发性痛风慢性肾损害(湿热血瘀证)的临床疗效。方法:对28例患者采用清热化湿活血法(处方:苍术、生地黄、豨莶草、赤芍、怀牛膝、槐花、土茯苓、萆薢、丹参、白花蛇舌草、薏苡仁、徐长卿、地鳖虫)治疗,辅助降压药物控制血压。观察临床症状、血压、血红蛋白、血尿酸、尿N-乙酰-β-D氨基葡萄糖酶、血肌酐等的变化情况结果:临床症状改善早于肾功能的改善;头晕目眩症状与血尿酸值平行改变;各阶段实验室指标和证候积分差值变化有显著性或非常显著性意义(P<0．05,P<0．01)。结论:清热化湿活血法治疗原发性痛风合并慢性肾损害(湿热血瘀证)Objective: To explore the clinical effect of therapy of Qingre Huashi Huoxue (TQHH) for primary gout complicated with chronic renal damage (damp-heat and blood-stasis syndrome). Methods: 28 cases were treated by TQHH agreement formula (composed of Radix A-tractylodis, Radix Rehmanniae, Herba Siegesbeckiae, Radix Paeoniae Rubra, Radix Achyranthis Bidentatae, Flos Sophorae, Rhizoma Smilacis Clabraev Rhizoma Dioacoreae Septemlobae, Radix Salviae Miltiorrhizae, Herba Hedyotis Diffusae, Semen Coicis, Radix Cynanchi Paniculati and E-upolyphaga seu Steleophaga) and associated with hypotensive to control blood pressure. 8 weeks constituted the course of treatment. The parameters of syudrome pattern, clinical symptoms, blood pressure, hemoglobin, serum uric acid, uric N - acetyl -β- D - glucosaminidase and serum crea-tinine were observed. Results: The improvement of clinical symptoms was earlier than the improvement of renal function, the change of dizziness and uric acid value were parallel, various parameters in different periods of laboratory test and the changes of scoring of syndrome pattern were marked or very marked significant difference. Conclusion: TQHH possesses reliable curative effect for primary gout complicated with chronic renal damage.
