摘  要:目的探讨肺不张的超声影像诊断及意义。方法对连续选择的763例胸内外科患者行同期盲法胸部CT和超声检查。与CT结果作对照,观察超声对肺不张的检出情况。结果超声对肺不张的敏感性100%(120/120).特异性100%(643/643),对不张肺叶定位的符合率100%。肺不张的声像表现:(1)病变肺叶呈-尖端指向肺门、范围小于正常体表投影、且与肝脏回声相近似的楔形实质回声区;(2)纵隔患侧移位;(3)伴有胸水时,相应范围内见到外周为液体包绕、内侧与纵隔相延续、回声强度视胸水多少而定的楔形或略有变异的实质回声区。结论作为一种观察肺不张的手段,超声影像诊断行之有效,可作为当前影像手段的一种补充,供临床酌情选用。Objective To investigate applying value of ultrasonography in atelectasis. Methods Among 763 patients,one hundred and twenty patients with atelectasis were examined by CT and ultrasoud in a prospective double-blind randomized concurrent controlled trial in turn. Results The both of sensitivity and specificity of ultrasnography to the atelectasis were 100 % ;The accuracy of diagnosis to which the lobe collapsed was 100 %. The typical appearances of atelectasis as follows,(l )Simple type:the region of atelectasis had a fan-like shape, some ducts that distributed from the lung hilus to the circumference radially were seen;The mediastinum moved to the ill side. (2) Pleural effusion type:the atelectasis area,of which the shape was fan-llke or variant, was encircled by the pleurao effusion and the inner side of it was continuous with mediastinum. Conclusion The ultrusonography can be served to diagnose atelectasis.
