中性粒细胞凋亡调控基因表达异常在全身炎症反应综合征中的作用

摘　　要：目的观察全身炎症反应综合征（SIRS）时中性粒细胞（PMN）凋亡的异常变化及其凋亡调控基因的表达情况，评价其临床意义。方法8例SIRS患者（均为急性胰腺炎患者）作为SIRS组。6名健康献血者作为正常对照组。分别观察两组外周血PMN凋亡率、凋亡调控基因Fas／FasL和凋亡信号转导分子天冬氨酸特异性半胱氨酸蛋白酶-3（caspase-3）及血清白细胞介素-6（IL-6）、IL8的水平。结果SIRS组外周血血清IL-6和IL-8水平均明显高于正常对照组（P均＜0．01）。SIRS组患者外周血PMN细胞凋亡率较正常对照组明显减少（P＜0．01）；两组PMN在体外培养24h后，用蛋白质免疫印迹法（Western blotting）没有检测到FasL表达；SIRS组患者外周血PMN的Fas、caspase-3表达水平与正常对照组比较明显降低（P均＜0．01）。结论SIRS患者存在PMN的凋亡异常和Fas、caspase-3表达水平下调，PMN凋亡延迟在SIRS的发生发展中有着重要意义。Objective To study the role of the polymorphonuelear neutrophil （PMN） apoptosis and expression of Fas and caspase - 3 in the systemic inflammatory response syndrome （SIRS）. Methods Eight acute pancreatitis patients with SIRS, 6 healthy control subjects were enrolled to study apoptosis of PMN in peripheral blood samples, expression of Fas/Fas ligand （FasL） and caspase -3 in the PMN, the levels of serum interleukin - 6 （IL - 6） and IL 8 were observed. Results Spontaneous apoptosis was significantly delayed in PMN from the SIRS patients with higher serum IL - 6 and IL - 8 levels compared with controls （both P〈0. 01）. PMN apoptosis rate in peripheral blood of patients with SIRS was lowered than that of controls （P〈0.01）. The expressions of Fas and caspase -3 in the peripheral circulating PMN were higher in the controls than those in the SIRS patients （both P〈0. 01）. Serum FasL expression was not found by Western blotting 24 hours after culture of PMN in vitro. Conclusion Peripheral circulating PMN from acute pancreatitis patients with SIRS show delayed apoptosis, decreased expressions of Fas and caspase - 3, and prolonged PMN survival may contribute to the development of systemic inflammatory injury characteristic of SIRS.
