摘  要:目的应用定量组织速度成像技术(QTVI)研究2型糖尿病(T2DM)患者微量白蛋白尿与左心收缩功能的相关性。方法60例T2DM患者按尿白蛋白排泄率(UAER)分为2组:正常白蛋白尿(NAU)组、微量白蛋白尿(MAU)组,正常对照组(NC)30例。收集3组研究对象的相关临床资料,应用QTVI获得左室侧壁、后间隔、前壁、下壁、前间隔、后壁二尖瓣环处的收缩期峰值速度(Vs),计算平均Vs(AVs),对这些参数进行组间比较及相关性研究。结果①MAU组空腹血糖(FPG)、餐后2h血糖(PPG)、总胆固醇(TC)高于NC(P<0.01)。②MAU组左室六个室壁Vs、AVs较NC组降低(P<0.01)。③通过线性逐步回归分析可知T2DM患者微量白蛋白尿可以作为二尖瓣环AVs的独立影响因素。结论微量白蛋白尿不仅是2型糖尿病肾病的早期敏感诊断指标之一,同时也是左心收缩功能受损的重要预报指标之一,且是独立影响因素。Objective To evaluate the relationship between Left ventr-icular systolic function and albuminuria in type 2 diabetic patients by quantitative tissue velocity imaging. Methods 60 type 2 diabetic patients and 30 norreal controls(NC) were included. On the basis of urinary albumin excretion rate (UAER), patients were classified into : no albumi-nuria ( n = 30 ), microalbuminuria ( n = 30 ). The correlated clinical information and the systolic peak velocity(Vs) of 6 mitral annulus sites by QTVI were measured. Results 1. The plasma levels of FPG,PPG ,TC in MAU group were higher than the NC(P 〈0. 01 ). 2. The Vs and 6 - site average Vs(AVs) in microalbuminuria group were lower than the NC (P 〈 0.01 ). 3. In multi-variate analysis, albuminuria status remained independently associated with AVs. Conclusion The left ventricular systolic function in type 2 diabetic patients was impaired in microalbuminuria group and microalb-uminuria was independently associated with it.
