摘 要:目的了解阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)儿童腺样体组织中糖皮质激素受体(glucocorticoid receptor,GR)亚型mRNA表达。方法34例儿童行腺样体切除,术前睡眠监测;应用荧光定量逆转录PCR法检测患者腺样体组织中GR-d和GR-BmRNA的表达。结果腺样体组织中均存在GR-d和GR-8的mRNA表达,且GR-d mRNA呈高表达。OSAHS患儿(22例)腺样体组织中的GR-d和GR-B mRNA(x±s,以下同)分别为(9.404±3、06)×10^5cDNAcopies/μg和(1.574±0.3)×10^4cDNA copies/μg;非OSAHS患儿(12例)腺样体组织中的GR-α和GR-βmRNA分别为(1.604-0.26)×10^6 cDNA copies/μg和(1.524±0.18)×10^4cDNAcopies/μg。非OSAHS患儿腺样体组织中的GR-α mRNA较OSAHS患儿腺样体组织中的GR-α mRNA增高(F=40.285,P〈0.001);OSAHS患儿腺样体组织中的GR-β mRNA较非OSAHS患儿腺样体组织中的GR-β mRNA稍高,但差异无统计学意义(F=0.145,P=0、706)。OSAHS、非OSAHS腺样体组织中GR-α/GR-β比值分别为62.34-20.3和107、44-24.4,差异有统计学意义(F=33.059,P〈0.001)。结论OSAHS儿童腺样体组织中存在糖皮质激素受体亚型表达,非OSAHS儿童腺样体对糖皮质激素可能比OSAHS儿童更敏感。GR在儿童OSAHS中的作用有待进一步研究。Objective The expression of glucocorticoid receptor (GR) expression in obstructive sleep apnea hypopnea syndrome (OSAHS) in children is currently unknown. The aim of this study was to determine the GR-α and GR-βstatus in the adenoidal tissues in children with OSAHS. Methods Thirty-four podiatric patients (aged 3-14 years, median 7.8 years ) had sleep study with polysomnography before adenoidectomy. According to the criteria of apnea hypopnea index (AHI) ≥5/h or/and apnea index (AI) ≥1/h, they were divided into OSAHS and non-OSAHS sub groups. The study was based on fluorescent quantitative PCR(FQ-RT-PCR)for the mRNA expression of GR-α and GR-β in the adenoidal tissues in children. Results GR isoforms mRNA encoding for expression of both GR-α and GR-β were detected in the adenoids of all children. GR-α mRNA level [ (9. 40 ±3.06) × 10^5 eDNA copies/μg total RNA] in the adenoidal tissues in OSAHS was lower than those in the non-OSAHS[ ( 1.60 ±0. 26) × 10^6 cDNA copies/μg total RNA ] ( F = 40. 285, P 〈 0. 001 ) , whereas no differences found for GR-β [ ( 1.57 ± 0. 35 ) ×^ 104 cDNA copies/μg total RNA, ( 1.52 ± 0. 18 ) × 10^4 cDNA copies/μg total RNA ]. GR-α/GR-β ratio was 62. 3 ± 20. 3 in OSAHS and 107.4 ±24. 4 in non-OSAHS. AHI or AI was not related to the mRNA levels of GR-α and GR-βin OSAHS or non-OSAHS. Conclusions GR-α and GR-β were detectable in the adenoidal tissues in children. These data indicated that the relationship between the expressions of GR and the clinical significance in OSAHS need further and profound investigation.
