摘 要:目的 探讨骨髓干细胞(bone marrow mesenehymal stemcell,MSC)移植对慢性心力衰竭大鼠模型心肌瘢痕周边及远隔区心肌的影响。方法 制作心力衰竭大鼠模型,4周后分为细胞移植组(将5×10^6MSC注射到梗死后心肌)和对照组(注射等体积磷酸缓冲液),每组7只。组织多普勒评价室壁运动。Tunel染色检测心肌细胞凋亡情况。RT-PCR及Western杂交检测Bax、Bcl2基因和蛋白表达。结果细胞移植组在心肌瘢痕区观察到血管形成。与对照组相比,治疗后21d移植组大鼠心肌收缩及舒张期纵向峰值速度在左室前壁、后壁、前室间隔及二尖瓣水平均显著升高[左室前壁收缩峰值速率为(0.67±0.02)cm/s对(0.31±0.02)cm/s,P〈0.001];左室前壁舒张峰值速率为(1.26±0.04)cm/s对(0.4±0.01)cm/s,P〈0.001];移植组大鼠心肌凋亡细胞减少[(9.2±1.1)个/mm^2对(28.5±3.6)个/mm^2,P〈0.001];瘢痕远隔区细胞比对照组细胞减小[(29.5±1.6)μm^2对(78.5±3.9)μm^2,P〈0.001]。与对照组相比,移植组大鼠心脏Bax基因和蛋白表达降低,Bcl2基因和蛋白表达升高。结论 MSC移植改善瘢痕边缘缺血心肌血供,增强缺血心肌功能;拮抗细胞凋亡,防止远隔区细胞肥大。Objective To elucidate the effect of marrow stromal cell (MSC) transplantation on border zone and remote region around scar of SD rats with heart failure (CHF) due to myocardium infarction. Methods SD rats with post-ischemic congestive heart failure were randomly divided into 2 groups: group M (n=7) and group C (n=7). All rats underwent doppler tissue imaging (DTI) to assess left ventricle (LV) recovery of function and ventricle motion. Apoptosis was tested by TdT- mediated dUTP-biotin nick end labeling (TUNEL). Bax and Bcl2 expressions were evaluated by reverse transcriptase PCR (RT-PCR), immunohistochemistry and western blotting. Results After MSC transplantation, peak E velocity and peak S velocity were significantly increased at posterior wall, anterior wall and anterior interventricular septum. The ameliorated regional myocyte hypertrophy in remote zone could be observed[myocyte size, (29. 5±1.6) μm^2 in group M versus (78. 5±3. 9) μm^2 in group C, P〈0. 001]. Apoptosis cell number decreased in group M vs. in group C[(9.2±1.1) count/mm^2 versus (28. 5±3.6) count/mm^2 , P〈0.001]. Compared with group C, Bcl2 gene and protein expressions increased in group M, while Bax gene and protein expressions decreased. Conclusions MSC transplantation prevents cell apoptosis and reverse myocyte hypertrophy in remote zone.
