摘 要:目的探讨钙磷骨水泥(calcium phosphate cement,CPC)复合重组人血管内皮细胞生长因子(recombinant hu-man vascular endothelial growth factor,rhVEGF)+重组骨形态发生蛋白-2(recombinant human bone morphogenetic protein-2,rhBMP-2)促进大段异体脱蛋白骨(deproteinized bone,DPB)移植再血管化的能力。方法成年杂交犬36只,距关节面30mm切除右侧股骨下端制成半关节缺损模型,分成CPC/rhVEGF/rhBMP-2/DPB移植组(A组);rhVEGF/rhBMP-2/DPB移植组(B组);单纯的DPB移植组(C组)3组。于术后4、8、12、16周行X线检查、组织学检查、透射电镜检查、墨汁灌注微血管分析、放射性核素骨显像(ECT)检测,观察血管发生和新骨形成情况。结果A组在各时间点X线及组织学,其成骨能力和血管生成均优于B、C两组,血管墨汁灌注图像分析和ECT检测动脉灌注情况均优于B、C两组(P<0.01),B、C组间在各时间点差异无统计学意义(P>0.05)。结论CPC/rhVEGF/rhBMP-2/DPB可促进异体大段骨移植的成骨能力及局部血管早期形成,使移植体最终达到骨愈合成为自身组织。Objective To explore the ability of revascularization of big section deproteinized bone (DPB) allograft combining calcium phosphate cement (CPC) with recombinant human vascular endothelial growth factor (rhVEGF) and bone morphogenetic protein-2 (rhBMP-2). Methods Totally 36 adult hybrid dogs were inflicted to demirticular defect models by exsecting 30 mm right femur inferior extremity near articular surface, and then randomly divided into 3 groups, group A, receiving material CPC/rhVEGF/rhBMP-2/DPB transplantation into the demi-articular defect, group B, material rhVEGF/rhBMP-2/DPB transplantation, and group C with simple material DPB transplantation. At 4, 8, 12 and 16 weeks after operation, vasculogenesis and new bone formation were assessed by X-ray examination, histological examination, transmission electron microscopy, vas capillare analys The bone formation suits of vaso-filling is by vaso-filling with ink and radionuclide bone imaging examination (ECT). Results and vascularization was significantly superior in group A to that in group B and C. The reexamination with ink and arterial perfusion of ECT examination were both significantly superior to that in group B and C (P 〈0.01 ). But there was no significant difference between group B and C. Conclusion The materials CPC/rhVEGF/rhBMP-2/DPB can promote the bone formation and early formation of local blood vessels, even achieve bone healing and become autologous tissue.
