McKenzie技术治疗腰痛临床效果的综合评价

摘　　要：目的:运用综合评价法,比较McKenzie技术和中医推拿手法治疗腰痛的疗效。方法:对70例腰痛患者(皆为“可减轻的移位综合征”),使用McKenzie技术治疗35例,使用中医推拿手法治疗35例。对治疗前和每天治疗后采用“视觉模拟评分法”(VAS)所得的疼痛评分,及对腰椎活动度、下肢放射痛指标,治疗5天后的有效率进行比较分析,并在比较临床效果各分指标的基础上引进综合评价模型对两组治疗效果进行综合分析。结果:治疗前,McKenzie组的VAS疼痛评分(7.84±1.26)和中医推拿组的评分(7.67±1.12)组间比较,无显著性差异P>0.05,经1—5天治疗后,各天McKenzie组的VAS疼痛评分均较中医推拿组低(P均<0.05),说明McKenzie组的疗效较好。在腰椎前屈和后伸功能及下肢放射痛的改善上,McKenzie组也明显好于中医推拿组(P均<0.01)。用综合指数评价法进行综合分析,治疗前两组比较无显著性差异(Z=0.07,P>0.05);治疗5天后两组比较有显著性意义(Z=7.13,P<0.001),综合指数McKen-zie组的平均水平好于中医推拿组。结论:McKenzie技术治疗腰部力学性疼痛效果显著,见效较快。Objective： Comprehensive evaluation was used to compare the clinical treatment effect of McKenzie therapy with that of traditional massage on the low back pain patients.Method： Seventy low back pain patients （the reducible derangement syndrome） were involved in our study. They were divided into two groups： In the first group, 35 cases received McKenzie therapy; in the second group, other 35 cases had traditional massage. Before each treatment sesslon,we recorded the pain scores using visual analogue scale（VAS）,the ROM of the lumbar spine and the symptom of radiation pain of the leg. Treatment effective rate was collected and recorded. After ward,we used the model of comprehensive evaluation to compare the effect between the two approaches. Result：Before the treatment,the VAS scores were 7.84±1.26 and 7.67±1.12 respectively in patients receiving McKenzie therapy and traditional massage,there was no significant difference in the scores of VAS between the two approaches. A significant difference was found in the McKenzie when compared with the traditional massage in each session for total 5 sessions （P〈0.05）. The ROM of lumbar and the radiation pain of leg were better in McKenzie than in traditional massage（P〈0.01）. By using the comprehensive evaluation,the comprehensive index of the two groups did not demonstrate a significant difference before session（Z=0.07 ,P=-0.94）. To compare the two groups after 5 times of treatments, the group of MeKenzie was better than the group of traditional massage as （Z=7.13,P〈0.001）,which had statistical significance.Conclusion：The McKenzie therapy is a better method than the traditional massage to treat the low back pain.
