肝康栓联合干扰素治疗慢性乙型肝炎

摘　　要：目的:评价干扰素(IFN)联合肝康栓治疗慢性乙型肝炎的临床疗效。方法:慢性乙型病毒性肝炎患者106例,分为A组52例,B组54例,均每日肌肉注射IFN(IFN-α)3×106U,4周后改为隔日1次。A组同时联合应用肝康栓栓剂,早晚1粒塞肛,治疗期间不再接受其他护肝药物治疗;B组治疗期间根据患者肝功能情况可辅助以甘草酸二胺(甘利欣)胶囊口服治疗。疗程均为6个月。结果:①生化指标,治疗后第1个月时与治疗前比较,2组患者均明显好转(P<0.05),自第2月起基本恢复正常。②HBsAg、HBeAg的阴转率,治疗第3个月及第6个月时,A组明显高于B组(P<0.05),HBVDNA水平2组均显著下降(P<0.05),A组较B组更明显(P<0.05)。复发率比较A组显著低于B组(P<0.05)。③血清INF-αI、L-6及IL-2水平与治疗前比较,2组INF-αI、L-6下降,IL-2上升,A组较B组更明显(P<0.05)。结论:IFN联合肝康栓能有效恢复患者肝功能,增强抗病毒效应,降低复发率,这可能与肝康栓参与调节机体免疫系统有关。Objective： To investigate the effectiveness of interferon combined with Gankang suppository in the treatment of chronic hepatitis B （CHB）. Methods： 106 cases of CHB were divided into 2 groups randomly： IFN group （54 cases） and IFN＋ Gankang group （52 cases）. The patients in IFN group were subjected to IFN-α（3× 10^4 U）, and those in IFN＋Gankang group to IFN-α combined with Gankang. The treatment duration was 6 months. Results： The liver function was improved in both groups one month after the treatment. From 2nd month the liver function recovered to normal level in both groups. The rate of HBsAg and HBeAg negative change was higher in IFN＋Gankang group than in IFN group （P〈0.05）. The recurrent rate in the IFN＋Gankang group was lower than in IFN group （P〈0.05）. The level of TNF-α and IL6 was decreased after the treatment in both groups. But the level of TNF-α and IL-6 was lower in IFN＋Gankang group than in IFN group （P〈0.05）. And the level of IL-2 was increased in both groups after treatment, but it was lower in IFN＋Gankang group than in IFN group （P〈0. 05）. Conclusion： Liver function can be improved effectively after combined use of IFN and Gankang. Gankang suppository can improve the anti-virus effect of IFN and inhibit the recurrence.
