超声造影定量分析在浅表淋巴结疾病鉴别诊断中的应用

摘　　要：目的探讨浅表淋巴结疾病超声造影时间-强度曲线的变化特征。方法对39例患者的65个浅表肿大淋巴结(良性23个,转移性35个,恶性淋巴瘤7个)进行灰阶超声造影,分析造影时间-强度曲线。结果良性组和淋巴瘤组淋巴结造影时间-强度曲线形态上表现为上升支陡直,到达顶峰时先快速下降,随后缓慢下降。转移性组淋巴结曲线形态上表现为上升支陡直,到达顶峰时持续缓慢下降。良性组和转移性组造影剂的到达时间及达峰时间无显著差异(P均>0.05);但良性组峰值强度大于转移性组(P<0.05)。淋巴瘤组到达时间短于良性组和转移组(P均<0.001);达峰时间短于良性组和转移组(P<0.05和P<0.001);峰值强度大于良性组和转移组(P均<0.001)。结论超声造影时间-强度曲线定量分析显示淋巴结疾病的不同灌注情况,对临床鉴别诊断有一定价值。Objective To study the characteristics and changes of time-intensity curves in superficial lymphadenopathy using gray-scale contrast-enhanced ultrasound. Methods Sixty-five superficial lymph nodes from 39 patients were studied by gray-scale contrast-enhanced ultrasound. Of the lymph nodes examined, 23 were benign and 42 were malignant （35 metastases and 7 non-Hodgkin lymphomas） . The time-intensity curves were evaluated by using Axius^TMauto-tracking contrast quantification （ACQ） . The diagnosis obtained by means of surgical or core biopsy. Results The curve appeared quick-up and quick-down type in the benign and lymphoma groups, but quick-up and slow-down in the metastatic group. There were no significance in the arrival time （AT） and time-to-peak （TTP） between benign and metastatic groups （all P〉0. 05）; but the peak intensity （PI） in the benign group was significantly higher than that in the metastatic group （P〈0.05） . The AT and TTP in lymphoma were significantly lower than those in the benign （P〈0.001 and P〈0.05） and metastatic （both P〈0.001） groups; and the PIs in lymphoma were significantly higher than those of the benign and metastatic groups （both P 〈 0. 001 ） . Conclusions With the characteristics of time-intensity curves and acoustic quantified parameters using gray-scale contrast-enhanced ultrasound, we can do better in differentiations of superficial lymphadenopathy,
