摘  要:目的:了解雄激素与子宫肌瘤发生的关系,探讨雄激素治疗子宫肌瘤的机制。方法:术前研究组服甲睾酮10mg,每日1次,连用1个月。对照组不用任何药物。检测研究组及对照组子宫局部及外周血FSH、LH、E2、P、T水平。用免疫组化S-P法检测子宫肌瘤、肌层、内膜ER、PR、AR的表达。结果:研究组及对照组子宫局部及外周血FSH、LH、E2、P、T无明显差异(P>0.05)。研究组血清FSH、LH、E2、P水平明显低于对照组(P<0.05或P<0.01)。对照组在增殖期肌瘤ER表达明显高于同一子宫肌层(P<0.01)。PR、AR表达在肌瘤与肌层间无差异(P>0.05),AR表达水平低。研究组ER表达明显低于对照组(P<0.01),PR、AR表达与对照组无明显差异(P>0.05)。结论:雄激素与子宫肌瘤的发生可能无直接相关。雄激素是通过抑制下丘脑-脑垂体-卵巢功能,使E2、P分泌减少,而达到缩小子宫肌瘤的效应。OBJECTIVE: To study the association between androgen and myomagenesis of uterus and to discuss the mechanism of androgen in treating uterus myoma. METHODS: Patients in trial group were administered preoperatively with 10mg methyhestosfer q.d. for 1 month, while those in control group were not given any medicine; luteinizing hormone (LH), follicular stimulating hormone (F'SH), estradiol (E), progesterone (P) and testosterone (T) levels in uterus and peripheral blood in both trial group and control group were determined. The expression - s of estrogen receptors, progestoren receptors and androgen receptors in uterus myoma, myometrium and endometrium were determined by immunohistochemical S - P method. RESULTS: No significant differences were noted between trial group and control group in the expression - s of FSH, LH. E2, P and T in uterus and peripheral blood ( P 〉 0.05). Serum FSH, LH, E2 and P were significantly lower in trial group than in control group ( P 〈 0.05 or P 〈 0.01 ). The expression - of ER at proliferative phase in the control group was higher in uterus myoma than in myometrium of the same uterus ( P 〈 0.01 ), but the expression - s of PR and AR in uterus myoma and myometrium were of no significant differences ( P 〉 0.05 ) and the expression - of AR was very low in both group. The expression - of ER was significantly lower in trial group than in control group ( P 〈 0.01 ), but the expression - s of PR and AR in trial group and control group were of no significant differences ( P 〉 0.05). CONCLUSION : There is no direct correlation between androgen level and the myomagenesis of uterus; The mechanism of androgen in diminishing the size of uterus myoma may be attributed to its inhibition on hypothalamus - pituitary - ovarian axis (HPO)thus decreasing the secretion of E2 and P.
