摘  要:目的观察丁咯地尔对豚鼠噪声性听力损伤的影响。方法将13只豚鼠随机分为丁咯地尔组(5只),噪声损伤组(5只),正常对照组(3只)。采用105 dB SPL(sound presure level,声压级)白噪声刺激,连续5 d(5 h/d)制造噪声性耳聋动物模型,其中丁咯地尔组在每天噪声刺激前30 min给予丁咯地尔(200 mg/kg)灌胃。豚鼠在给噪前后进行听性脑干诱发电位(ABR)和畸变产物耳声发射(DPOAE)检测,给噪后采用形态学方法观察耳蜗外毛细胞受损情况。结果噪声损伤组及丁咯地尔组豚鼠噪声刺激前后ABR阈值分别为(17.50±3.80)dB SPL、(16.00±3.94)dB SPL和(67.08±6.20)dB SPL(、49.00±4.59)dB SPL,两组豚鼠经噪声刺激后阈值的升高与正常组相比,差异有统计学意义(均P<0.01);DPOAE幅值的下降也具有统计学意义(均P<0.05)。而丁咯地尔组豚鼠ABR阈值的升高和DPOAE幅值的下降与噪声损伤组相比,差异也具有统计学意义(均P<0.05)。耳蜗基底膜铺片观察到凋亡、坏死和缺失的受损外毛细胞,计算其受损率发现,丁咯地尔组豚鼠外毛细胞受损率明显低于噪声损伤组(P<0.01)。结论预防性使用丁咯地尔可以减轻噪声对豚鼠耳蜗的损害,对噪声引起的听力损伤可能有一定的保护作用。Objective To study the protective effect of Buflomedil on noise-induced hearing loss in guinea pigs. Methods Guinea pigs were randomly divided into Buflomedil-treated group (receiving 200 mg/kg Buflomedil per day), white noise (WN) group and normal control group. The animals in Buflomedil-treated group and WN group were exposed to WN at 105 dB SPL for consecutive 5 days (5 h per day) and those in normal control group not subjected to any noise. Auditory brainstem response (ABR) thresholds and distortion product otoacoustic emission (DPOAE) were monitored before and after noise exposure, and the organs of corti were processed for detection of hair cell injury after noise exposure. Results The ABR thresholds in WN group and Buflomedil-treated group before and after noise exposure were (17.50±3.80), (16.00±3.94) and (67.08±6.20), (49.00±4.59) dB SPL respectively. The auditory thresholds were significantly increased (all P〈0.01) and DPOAE level decreased (all P〈0.05) in WN group and Buflomedil-treated group as compared with those in normal control group. Laser scanning confocal microscopy showed that the outer hair cells displayed apoptosis, necrosis and missing with the difference being significant between WN group and Buflomedil-treated group. Conclusion Preventive use of Buflomedil could alleviate noise-induced damage to outer hair cells in guinea pigs, and could protect against noise-induced hearing loss to some extent.
