非糖尿病科住院的2型糖尿病患者血糖控制状况分析

摘　　要：目的 探讨非糖尿病科住院的2型糖尿病患者血糖控制状况。方法 以我院非糖尿病科2003年11月～2004年10月收治的123例伴有2型糖尿病患者的临床资料为研究对象。回顾性分析患者出、入院时血糖控制状况、血糖监测率、治疗方案、内分泌科会诊情况和出院医嘱。结果 出院时，患者空腹血糖监测率及空腹和餐后血糖同时监测率显著低于入院时（P〈0．05），空腹血糖控制情况与入院时比较差异无显著性意义（P〉0.05）；出院时餐后血糖监测率和达标率与入院时比较差异无显著性意义（P〉0.05），但餐后血糖尚可率显著高于入院时，控制差率显著高于入院时（P〈0.01）。结论 非糖尿病科医师对糖尿病患者血糖的监测和控制很不规范，这对糖尿病的正规治疗和糖尿病慢性并发症的有效防治非常不利。Objective To investigate the situation of blood glucose control in inpatients with type Ⅱ diabetes mellitus in non - diabetic department. Methods The clinical data of 123 hospitalized patients with type Ⅱ diabetes mellitus in non - diabetic medical department in our hospital from November 2003 to October 2004 were taken as the subjects. The conditions of inpatients with type Ⅱ diabetes mellitus were retrospectively analyzed, including the situations of blood glucose control, monitoring rate of glycemia, therapeutic planning, consultation in department of endocrinology and doctor＇s advices on discharge. Results On discharge the patients＇ monitoring rate of fasting blood glucose （FBG） , and simultaneity of the FBG and postprandial blood glucose （PBG） were obviously lower than those on admission （ P 〈 0. 05） , No significant difference in FBG control was found between on the admission and the discharge （ P 〉0. 05） ; so was in PBG monitoring rate and optimal rate （ P 〉0. 05）. However on discharge, the PBG fair rate was higher, and poor rate was lower as compared with the corresponding rates on admission （ P 〈 0. 01 ）. Conclusion The situation of blood glucose monitoring and control for diabetics by non - diabetic specialists was very poor. That was harmful to the diabetic specialist therapy and the efficient prevention and treatment of chronic diabetic complicaltions.
