煤工尘肺的影像学特征对其院内获得性支气管-肺感染发病的影响

摘　　要：目的了解煤工尘肺(coal workers’pneumoconiosis,CWP)不同影像学特征对院内获得性支气管—肺感染(hospital ac-quired pulmonary lnfection,HAPI)的影响。方法采用回顾性的研究方法,观察HAPI在不同小阴影类型及尘肺期别中的分布特点。结果HAPI在类圆形小阴影、不规则阴影煤工尘肺患者中的分布不同(P<0.05);在Ⅰ期CWP中,类圆形、不规则小阴影发生HAPI的分布不同(P<0.05);不同小阴影类型p、q、r、s、t、u之间CWP患者发生HAPI分布,q与t比较,P<0.05,其余两两间无差异(P>0.05);Ⅱ、Ⅲ期煤工尘肺类圆形、不规则小阴影发生HAPI的分布无差异(P>0.05);不规则小阴影类型CWP中,不同尘肺期别发生HAPI分布无差异(P>0.05);类圆形小阴影p类型CWP中,不同尘肺期别发生HAPI分布无差异(P>0.05);发生HAPI与未发生HAPI的CWP患者小阴影类型构成比无差异(P>0.05);发生HAPI与未发生HAPI的CWP患者尘肺期别构成比无差异(P>0.05)。结论不规则小阴影较类圆形小阴影的CWP患者更易患HAPI;随着尘肺期别的升高,不规则小阴影类型对CWP患者HAPI的影响减弱;未观察到尘肺期别对HAPI的影响。[ Objective] To learn the effect of various radiograph manifestation of the coalworkers; pneumoconiosis （ CWP） on the hospital acquired pulmonary infection （ HAPI ）. [ Methods ] Retrospective method was applied to observe the distribution features of HAPI in various small opacity and pneumoconiosis stages. [ Results] Different distribution of CWP with HAPI was found in irregular small opacity and similar roundness opacity （ P 〈 0. 05 ）, so was found in stage I of CWP （ P 〈 0. 025 ） ; among the small opacity p, q, r, s, t and u, significant difference was found between q and t （ P 〈 0. 05 ） but not between any other two types （ P 〉 0. 05 ） ; no significant difference was found between different CWP stages in either irregular small opacity and similar roundness opacity （ P 〉 0. 05 ） ; the constitution ratio also indicated no significant difference either in small opacity types and CWP stages between the CWP patients who had HAPI and who didnt have HAPI （P 〉0. 05）. [Condusion] CWP patients with irregular small opacity were found more vulnerable to HAPI ; the effect of the irregular small opacity on the CWP patients with HAPI descends with the increase of CWP stages; pneumoconiosis stages were found to have no effect on HAPI.
