外伤性截瘫患者生存质量与社会支持的相关性研究

摘　　要：目的探讨外伤性截瘫患者出院后生存质量与社会支持的关系，为提高患者的生存质量提供参考。方法采用世界卫生组织生存质量测定量表简表（WHOQOL-BRIEF）和社会支持评定量表（SSRS）对58例外伤性截瘫患者在出院后进行追踪调查，并对其生存质量与社会支持关系进行相关性分析。结果外伤性截瘫患者生存质量总分为48．81±23．92，社会支持总分为32．17±13．62，其生存质量与社会支持呈显著正相关（r=0．912，P〈0．01），其生理、心理、社会和环境维度与SSRS的10个务目之间呈显著正相关（r=0．603～0．915，P〈0．05、P〈0．01）；58例中，功能恢复较好者20例（34．48％），26例（44．83％）患者发生关节僵硬、肌肉萎缩、压疮、继发感染、骨质疏松等截瘫综合征，且症状呈加剧趋势。结论外伤性截瘫患者整体生存质量较差，社会支持较低，政府相关部门应关注患者的生存质量，建立良好的社会支持体系和社区护理服务网络及家庭支持，为其提供经济援助、信息服务、情感支持、心理咨询和康复指导。以提高患者的生存质量。Objective To probe into the relationship between life quality and social support of traumatic paraplegia patients after they leave hospital, and to offer information for further improvement of the life quality of the patients. Methods Checklist on Quality of Life issued by World Health Organization （WHOQOL-BRIEF） and Social Support Rating （SSRS） were used to conduct a survey in 58 discharged patients suffering from traumatic paraplegia. Results The total score of the patients with traumatic paraplegia was 48. 81± 23. 92 on average, the total score of society support was 32.17±13.62. The patients＇ life quality was positively correlated with the social support （r=0. 912, P〈0. 01）. A positive correlation existed between physiological, psychological, social, environmental survival quality and 10 items of SSRS（r=0. 603-0.915, P〈0.05, P〈0.01）. In the 58 cases, only 20 （34.48%） patients had good function recovery; 26 （44.83 %） suffered from so-called paraplegia sydrome including anehylosis, muscular atrophy, bed sore, secondary infection, osteoporosis, and the symptoms showed a tendency to aggravate. Conclusion The life qualily of patients with traumatic paraplegia is generally poor and social suppor is low. The governments should pay more attention to the life quality of the patients, introduce a well-established social support system and community nursing care network to provide the patients with financial support, information services, emotional support, psychological consultation and guidance in rehabilitation aimed at improving their life quality.
