肺心病呼吸衰竭患者血清和尿β_2-微球蛋白测定的意义

摘  要:用放免法测定62例肺心病并发呼吸衰竭患者的血清和尿β2—微球蛋白(β2—MG)水平,并以77例健康人为对照。结果表明肺心病并呼衰者血清和尿β2—MG明显高于对照组,且呼衰急发期明显高于缓解期(P<0.01)。血清β2—MG与Cr和BUN呈正相关(r=0.393和0.484,P<0.01),尿β2—MG与血Cr和BUN亦呈正相关(r=0.194,和0.217),但无统计学意义(P>0.05)同时还发现血清和尿β2—MG与PaO2呈负相关(r=-0.293,和-0.258,P<0.05),与PaC()。呈正相关(r=0.384,和0.357,P<0.01)。提示肺心病肾功能改变与呼衰有关,血清及尿的β2—MG水平可能是识别患者肾功能的较敏感指标。Serum and ucine β2-MG levels were determined in 62 patients with cor pulmonale who had respiratory failure and in 77 healths using radio immunoassay The results showed that serum β2-MG and urine β2-MG levels in the patients were much higher than that in control, and were also higher in acute phase of respi-ratory failure than that in remisslon phase. Statistical analysis revealed that serum β2-MG levels possitively correlated with Cr and BUN(r=0. 393,and 0. 484,P<0.01 ) and urine β2-MG levels also possitively correlated with Cr and BUN (r = 0.194,and 0. 217), but no signiflcance in statistics (P>0.05 ). The results also showed that serum β2-MG and urine β2-MG levels negatively correlated with PaO2(r=0.293, and -0. 258,P<0.05),and possitively correlated with PaO2 r=0. 384 and 0.357,P<0.01). We suggest that renal function changes in cor pulmonale correlate with respiratory failure,and β2-MG may be a sensitive marker for identifying re-nal dysfunction in cor pulmonale with respiratory failure.
