尘螨变应原疫苗治疗变应性鼻炎的免疫学观察

摘　　要：目的:探讨尘螨变应原疫苗治疗变应性鼻炎患者的疗效和免疫机理。方法:采用病例自身对照方法,比较35例尘螨变应性鼻炎患者在尘螨变应原疫苗脱敏治疗前后的症状积分,及血清IL-2,IL-4,IL-5水平。结果:35例患者全部坚持完一年脱敏治疗,治疗前后的症状积分分别为(9.63±1.61)分及(4.34±1.81)分,血清IL-2水平为(0.29±0.40)ng/L及(1.42±0.26)ng/L,血清IL-4水平为(75.40±8.67)ng/L及(46.09±8.68)ng/L,IL-5水平为(65.83±3.56)ng/L及(19.91±3.99)ng/L,均具有显著性差异(P<0.01)。35人中有11人出现局部不良反应,未出现1例全身不良反应。结论:尘螨变应原疫苗是治疗变应性鼻炎有效、安全的药物。可能通过增加血清IL-2水平,降低血清IL-4、IL-5水平起作用。Objective：To investingate the efficacy and immune mechanism of dust mite vaccine for the treatment of dust mite-allergic rhinitis patients.Methods：Self-contrast method was used to compare the symptom score,level of serum IL-2, IL-4 and IL-5 pre and post specific immunotherapy of the 35 dust mite-allergic patients.Results：35 patients completed the study.The symptom score was （9.63±1.61） and （4.34±1.81） pre and post therapy,level of serum IL-2 was（0.29±0.40）ng/L and （1.42±0.26） ng/L,level of serum IL-4 was （75.40±8.67） ng/L and（46.09±8.68） ng/L,level of serum IL-5 was （65.83± 3.56） ng/L and （19.91±3.99） ng/L,all the P values of the data were less than 0.01.11 of the 35 patients had local side effects only,and there was no systemic side effect appearing.Concluslon：Dust mite vaccine is an effect and safe drug for the dust mite allergic rhinitis patients.It may act by increasing the level of serum IL-2 and decreasing both the level of serum IL-4 and IL-5.
