摘 要:目的测定多囊卵巢综合征(PCOS)患者的胰岛素敏感性(IS),并分析其相关影响因素。方法将观察对象分为PCOS肥胖组(10例)、PCOS非肥胖组(28例)和正常对照组(9例),IS用两步法高胰岛素-正葡萄糖钳夹技术评估。结果(1)PCOS两组患者的葡萄糖利用率M值明显低于对照组(P均<0.01),肥胖的PCOS患者M值降低更明显。(2)多元线性回归分析显示游离睾酮及BMI与M值负相关(P<0.05)。结论非肥胖和肥胖PCOS患者均存在明显的胰岛素抵抗,PCOS患者IS与游离睾酮及BMI密切相关。Objective To measure insulin sensitivity in patients with polycystic ovary syndrome (PCOS) and analyze its associations with correlative factors. Methods The subjects were divided into three groups: obese PCOS(n= 10), non-obese PCOS(n= 28) and control group(n= 9). And the insulin sensitivity was assessed by two-step hyperinsulinemic-euglycemic clamp. Results (1) Insulin sensitivity (M value) was significantly lower in obese PCOS subjects than that in other two groups (all P〈0.01). The M value in obese PCOS subjects was even more lower than that in non-obese PCOS subjects (P〈0.05). (2) Stepwise regression analysis showed that M150-180 value was significantly associated with free testosterone (FT) and BMI. Conclusions Both non-obese and obese PCOS women have insulin resistance, and the insulin sensitivity of PCOS patients is negatively associated with serum FT and BMI.
